The progress of health care today depends to a large extent on the progress in pediatrics, the level of infant mortality and health indicators of children, especially in the first year of life. Therefore, it is no coincidence that the problems of perinatal pathology are among the highest priorities in modern medicine. Pediatric neurologists have been able to show and prove that a huge place in perinatology is occupied by birth injuries of the nervous system. Even minimal trauma, depending on which structures are damaged, can lead to irreversible changes up to and including death.

Experience in rehabilitation of orthopedic and traumatologic patients at the production association

This paper summarizes the experience of industrial rehabilitation of patients with orthopedic-traumatologic profile in the department of rehabilitation treatment of medical unit of "Izhmash" production association, which has been functioning since December 1983 as a part of a polyclinic and has an industrial rehabilitation section. The department includes water and mud baths, physical therapy, electrotherapy, mechanotherapy and massage therapy rooms. The industrial rehabilitation section includes three classrooms for assembling "Moskvich" car parts.

Myocardial contractile function and acid-base balance in trauma patients treated by the Ilizarov method under peridural anesthesia

Peridural anesthesia as a method of anesthesia continues to attract attention because it has no pathological effect on the functions of vital organs, except for rare cases of severe hypotension. There are controversial opinions regarding the effect of peridural anesthesia on cardiodynamics. Some authors believe that peridural blockade reduces the tonic action of sympathetic nerves, resulting in bradycardia, decreased myocardial contractile function with depressed cardiovascular system. Others argue that during peridural anesthesia there are no significant changes in intracardiac hemodynamics. According to others, moderate concentrations of anesthetic penetrating into the circulating blood increase cardiac output and slightly increase BP.

Therapeutic exercise in the complex treatment of victims of femur fractures

We have accumulated experience in the surgical treatment of diaphyseal fractures of the femur in 256 patients; closed intramedullary osteosynthesis was performed in 56 of them. Like a number of other authors, we performed this operation under the control of an electron-optical converter with bone marrow canal dissection. When the femoral canal is drilled out, part of the bone chips and bone marrow elements are embedded between the bone fragments, which improves the reparative process. This operation, which we perform usually on the 14th day after injury, does not damage the soft tissues in the fracture area, the periosteum and the femur itself. Fixation of bone fragments of the femur is performed using a powerful hollow metal nail with a slit, which, in our opinion, ensures stability of the fracture for the entire period of regeneration and saves the patient from additional external immobilization with a plaster cast with all its inherent drawbacks.

Bone grafting of limb bone defects after tumor removal

One of the indications for bone grafting is a bone defect resulting from the removal of bone tumors. This report describes the experience of bone grafting in 161 patients who underwent bone resection due to tumors and tumor-like processes of limb bones (Table 1). According to their age the patients were distributed as follows: 17 to 20 years old - 39 patients, 21 to 30 - 51, 31 to 40 - 46, 41 to 50 - 14, over 50 - 11.

Extrafocal compression osteosynthesis of mandibular fractures and its biomechanical justification

We studied the biomechanical features of mandibular fractures by mathematical modeling methods taking into account the data of direct and inverse gnathodinamometry. The forces of the masticatory muscles were measured strainsometrically. The following values of gnathodinamometry were taken for analysis of mathematical models of maxillofacial region: direct in central part - 21,28-28,28 kgf, in lateral parts - 37, 52-48,02 kgf; reverse - 7.26-11,16 kgf. Vectors of masticatory muscles and muscles of hyoid bone were studied mathematically.

Acute adhesive intestinal obstruction - retrospective analysis

206 patients with various forms of intestinal obstruction have been treated in our clinic during the last 5 years. In 78 (37,9%) patients intestinal obstruction causes were adhesions, in 46 (22,3%) - pinch hernia, in 40 (19,4%) - obturation, in 15 (7,3%) - volvulus, in 7 (3,4%) - spastic ileus, in 3 (1,5%) - paralytic ileus, in 3 (1,5%) - intussusception, in 14 (6,8%) - other causes. Out of 206 patients 39 died. Overall lethality in intestinal obstruction was 18.9%.

Diagnosis of recurrent rheumatic heart disease and its gradation in severity

The aim of the work was to develop diagnostic criteria for recurrent rheumatic heart disease and its variants differing in severity. To solve this question we used a complex of clinical, laboratory, instrumental data, as well as the results of pathological anatomical examination. WHO criteria with determination of streptococcal antibody titers were used during examination of soles. Besides routine methods of laboratory study, the state of body's immune system was evaluated: reaction of inhibition of leukocyte migration (RTML), titer of anti-streptococcal antibodies, measures of completed and incomplete phagocytosis, activity of blood serum acid phosphatase. The type and group of streptococcus were determined in nasopharyngeal smears. ECG and Echo-CG were recorded. The state of central hemodynamics was studied with mechanocardiography according to N.N. Savitsky.

Spa treatment of rheumatoid arthritis with the inclusion of dimexide electrophoresis

In order to optimize treatment, it is of interest to study the effectiveness of complex treatment with peloids and DMSO electrophoresis in rheumatoid arthritis patients. 75 patients aged from 19 to 55 years old with grade II rheumatoid arthritis according to the criteria of the American Rheumatology Association were under observation. The course of the disease was progressive, characterized by multiple joint lesions and lasted from 2 to 10 years.

Role of uterine cavity vacuum aspiration in the prevention and treatment of postpartum endometritis

Being supporters of early intervention, we have performed vacuum aspiration of the uterine cavity contents in 62 laboring women over the past two years to prevent the spread of infection beyond the endometrium. During this operation, unlike uterine curettage, the basal layer of the endometrium, the venous lacunas of the internal layer of the myometrium and the myometrium itself are not traumatized. The negative pressure created by the vacuum aspirator prevents the uterine contents from penetrating the venous network.

Antimicrobial effect of chlorhexidine bigluconate on the vaginal microflora

The aim of the work was to choose the optimal concentration of chlorhexidine bigluconate solution, effectively destroying the microflora that inhabited a woman's vagina, in the shortest time preventively cleansing it to the natural degree of purity (at least to grade II). Non-pregnant premenopausal women treated at the 2nd OB/GYN clinic of Kazan HIDUV for infertility, genital infantilism, ovarian sclerocystosis, etc. were under observation. Four groups of women were isolated and subjected to sanation with appropriate chlorhexidine bigluconate solutions in the following concentrations: 1, 2, 3 и 4%.

Radiological evaluation of laryngeal stenosis stage in acute laryngotracheitis in children

The study was conducted in 228 children in whom pneumonia was suspected during clinical examination, complicating the course of acute laryngotracheitis. There were 46 children aged 8 months to 1 year, 113 from 1 to 2 years, and 69 from 2 to 3 years. Clinically, acute respiratory failure in 111 children corresponded to stage I laryngeal stenosis, in 104 to stage II, and in 13 to stage III. A group of 10 children suffering from pneumonia without combination with acute laryngotracheitis was taken for comparison.

Polarographic activity of denatured serum filtrate and external respiration in acute pneumonia

In order to add new objective data to the existing diagnostic complex of pneumonia and its complications, as well as to monitor the course of treatment and make prognosis of the disease we performed a comprehensive comparative study of patients using polarographic and pneumotachographic methods. We followed up 56 patients with acute pneumonia (men - 36, women - 20), aged 15 to 66 years (mean age - 41 years). Pneumonia in 15 (26,8%) patients was without complications, in 25 (44,6%) it developed against the background of chronic bronchitis, in 5 (8,9%) it was complicated by abscessed; in 4 (7,1%) patients there were acute bronchitis, in 3 (5,4%) bronchocarcinomas and cancer metastases into the lungs were revealed, in 4 (7,2%) acute pneumonia occurred against the background of exacerbation of non-pulmonary somatic diseases. Control group consisted of 21 people aged 16-24 years.

To the diagnosis of pre-disease in smokers

Smoking is a risk factor for many diseases. In particular, it leads to the development of chronic bronchitis and its complications. However, in spite of the urgency of the research of cardiorespiratory system state in smokers, there are almost no data in the literature about the complex study of this system in this contingent. The aim of this work is to elucidate the mechanisms of early changes in the cardiorespiratory system and to develop methods for their diagnostics in smokers who consider themselves healthy. 325 smokers and 197 non-smokers were examined. Their medical history was studied, ECG, fluoro- and spirography (GEL, SPHF, Votchal-Tiffno index) were performed. Smokers considered themselves healthy, they did not have any complaints.

The value of bacteriological examination of ejaculate for the choice of treatment for male infertility patients

The aim of our study was to determine the pathogen supporting the inflammatory process in the genitals of infertile patients, its sensitivity to antibiotics and the effectiveness of treatment measures, carried out taking into account the data obtained by ejaculate culture. Eighty patients aged 21 to 33 years old with secretory toxic form of male infertility were examined. Clinical examination revealed signs of chronic inflammation in the prostate in 32 men, in the testicular appendages in 12 men, and chronic urethritis in 2 men. In 34 patients the inflammatory process had no clinical manifestations, but the ejaculate showed a large number of leukocytes.

On the issue of uroflowmetry in children

In the present work we consider uroflowmetry as a promising method of functional diagnostics in pediatric urological practice. Uroflowmetry is a method based on the measurement of volume urinary flow rate (VUR). The following parameters are recorded graphically: duration of urination, average urine flow rate, urine flow rate in the first second, time of reaching maximum urine flow rate, maximum urine flow rate and bladder volume.

Treatment of combined hand injuries

Combined hand trauma is observed in 85.5% of cases in persons under 50 years of age and leads to loss of professional ability to work in 41.8% of cases and to disability in 19-43% of cases. Therefore, treatment of concomitant hand injuries remains an urgent problem at the present time. Due to diagnostic errors, 61.3% of patients with median and ulnar nerve injuries refer to specialists only 3 months after injury. Simple diagnostic tests have been suggested: inability to oppose the I finger, inability to grasp a cylindrical object with the I finger, which is typical for median nerve injury. If a patient cannot cross II and IV fingers over or under III finger, it indicates ulnar nerve damage, and if he is unable to strike the table with half-bent fingers (II-V), it means the median and ulnar nerves are injured.

The attention of obstetricians and gynecologists to the method of reflexotherapy is attracted by the possibility of reducing the pharmacological load on the body of the pregnant woman and the fetus, the absence of unwanted side effects when used properly. Reflexotherapy can also be the method of choice in the presence of intolerance to a number of pharmacological compounds. Reflexotherapy improves protective properties of organism, has normalizing effect on tone and reactivity of vegetative section of nervous system, normalizes activity of endocrine glands, in particular, sex glands, regulates tone of muscles. It is proved that this method significantly influences hypothalamic-pituitary-adrenal system of the organism.

Prevention of occupational injuries among porcelain factory workers

We studied the injury rate among the workers of the Bugulma Porcelain Factory in 1982- 1987, taking into account the working conditions of the workers. The peculiarities of production at the plant include tedious, monotonous work on the conveyor belt at high temperature. The factory exists for 13 years. The young age of the workers - from 18 to 40 years - also draws attention.

Surgical treatment of the consequences of fractures of long bones with purulent infection

We have experience in applying different variants of Ilizarov osteosynthesis in the treatment of 158 patients and disabled persons with non-unionized fractures, false joints and defects of long tubular bones complicated by purulent infection. There were 116 men (73.4%) and 42 women (26.6%). The age composition ranged from 16 to 70 years. The majority (more than 60%) were of young working age (20 to 40 years). Open fractures of the tubular bones were previously detected in 148 (93.7%) patients, and closed fractures in 10 (6.3%). The latter patients developed postoperative osteomyelitis after osteosynthesis with various submersible metal fixators performed in rural district hospitals. The duration of the disease from the time of injury was from 5 months to 6 years.

Case of patellar arthroplasty

Knee fractures account for 1.5-2.2% of all skeletal injuries. While fractures of the patella without displacement are treated conservatively by applying a deep posterior plaster cast with a "drying" cast placed on the anterior surface of the joint, displaced fractures require surgical intervention. In the case of multiple comminuted and fractured patellar fractures with displacement, in which it is impossible to bring all fragments together using P-shaped lavsan or sutures, the best outcomes can be achieved by endoprosthesis, that is, removal of the entire fractured patella and replacing it with an endoprosthesis. In this report, we would like to share our experience with patellar endoprosthesis according to G.S. Samoilov performed for a multiaxial fracture in the hospital.

Method for assessing the functional state of the spine

On the treatment of purulent wounds

In 1985-1987, we treated 142 patients (77 male and 65 female) aged 1 to 85 years old with purulent wounds and purulent-necrotic lesions of soft tissues at the surgical department of the Central Regional Hospital. Pathogenic microflora isolated from wounds was found to be sensitive to kanamycin, ampicillin, streptomycin, monomycin, carbenicillin.

Processing echocardiography results with microcalculators

Ultrasound diagnostics in cardiology is associated with the necessity of time-consuming calculations of geometric parameters of the left ventricle. These calculations can be considerably reduced by the doctor using commercially available unfunded microcalculators, such as MK-56. Limited memory capacity of these microcalculators (99 commands) predetermined the creation of a library of self-applied programs.

Study of some ventilation parameters in patients with lung and heart diseases using a domestic volumospirograph

The most objective and sensitive method of bronchial obstruction diagnosis is determination of maximal expiratory flow at different levels of bronchial tree. Despite the lack of clear boundaries of the norm, the flow indices according to spirography allow early diagnosis of bronchial permeability disorders, which facilitates the diagnosis of obstructive bronchitis and bronchial asthma.

Lung surfactants in acute pneumonia

Lung surfactant system, according to modern views, is a multicomponent, complex, self-regulating system, in which there are 3 main components: the surfactant itself - a thin lipoproteid film located at the air-liquid interface, hypophase and a cellular component represented by alveolocytes of types I, II, III. Lung surfactant system provides stability of alveoli during breathing, supports normal permeability of aerohematic barrier, participates in regulation of water balance, and has a protective function. Surface-active components of surfactant system are phospholipids, and more precisely their fraction - phosphatidylcholine.

A case of in vivo diagnosis of generalized idiopathic amyloidosis

Idiopathic amyloidosis, unlike secondary amyloidosis, develops in the absence of previous disease. Its clinical presentation is multifaceted, determined by the lesion of certain organs and highly variable during the course of the disease. All manifestations of the disease are nonspecific. In contrast to secondary amyloidosis, idiopathic amyloidosis more often involves cardiovascular, respiratory, digestive and urinary systems. In vivo diagnosis of idiopathic amyloidosis is difficult due to its imitation of many diseases; in practice, it is recognized less often than it occurs.

Three cases of old uterine rupture

It is known that uterine ruptures are the most serious obstetric pathology; without timely surgical care they usually lead to death due to hemorrhagic and painful shock or sepsis. Life expectancy of such patients is usually a few hours, less often - a few days. There are single reports in the special literature about uterine ruptures, which ended with spontaneous "cure" - favorable outcome for a woman.

Conceptual framework for assessing quality and efficiency in health care

In our country during this period, spending on health care in relation to national income grew slowly, which led to a decrease in the corresponding share from 6.1% in 1960 to 4.6% in 1986. As of 1988, spending on health care was defined as 21.7 billion rubles, which is only 4.9% of national income. For comparison, in other economically developed countries this value reaches 8-12% of their gross national product.

Correction of the pelvic component in the surgical treatment of congenital dislocation of the hip in children

Over the past 8 years, the pediatric department of the All-Russian Scientific and Research Center "VTO" has been actively performing extra-articular surgeries for the treatment of congenital sub- and dislocation of the hip, aimed at correcting all elements of the hip joint deformity in one or two stages. Open reduction is usually combined with intervertebral osteotomy according to Pauwels. Pathological anteversion and valgus of the proximal femur are eliminated to normal age values. In order to prevent postoperative external-rotational contracture and reduce compression in the hip joint, we perform plasty of the external femoral rotators and the pelvic-trochanteric muscle group. We also actively apply pelvic component correction in children with an acetabular index of more than 30°. When the acetabular roof is slightly oblique in young children, we perform a Salter osteotomy.
