The experience of anticoagulation therapy has shown that control over the course and depth of action of anticoagulants by prothrombin time or prothrombin index is not entirely reliable. Cerebral hemorrhages, thrombosis and embolism, which are most often associated with atherosclerosis and hypertension, are among the main manifestations of vascular catastrophes, which, along with myocardial infarction, are the most serious in their prognosis.
Проблемы терапии нарушений мозгового кровообращения весьма актуальны. Мозговые геморрагии, тромбозы и эмболии, находящиеся чаще всего в связи с атеросклерозом и гипертонической болезнью, относятся к основным проявлениям сосудистых катастроф, которые, наряду с инфарктом миокарда, наиболее серьезны по своему прогнозу.
Лечению сосудистых заболеваний головного мозга посвящены работы многих отечественных и зарубежных авторов, большинство которых в последнее время подчеркивает целесообразность изучения процесса свертывания крови, определения содержания его отдельных компонентов и особенно протромбина.
Нами проводилось наблюдение за содержанием протромбина в крови у 77 больных с поражением мозговых сосудов (тромбоз — 23, эмболия— 7, атеросклероз—12, эндартериит — 9, кровоизлияние в мозг — 12 и с другими поражениями нервной системы—14). Мужчин было 47, женщин — 30. До 30 лет — 8, от 31 до 50 лет — 24, от 51 до 60 лет — 19 и старше — 26 чел.
При тромбозе сосудов головного мозга протромбин крови был повышен у 13 чел. (106—12Г%), в норме — v 5, понижен—у 5 (не ниже 76%).
Среди больных эмболией сосудов головного мозга протромбин был повышен у 4 (107—114%), в норме — у 2 и снижен — у 1 (86—94%).
У больных с кровоизлиянием в головной мозг протромбин был повышен у 7 (106—121%), в норме — у 3, понижен — у 2 (не ниже 72%);
Заслуживает внимания выраженная лабильность уровня протромбина в крови у больных мозговыми инсультами. Эта лабильность больше выражена у больных геморрагиями и тромбозами сосудов головного мозга. При склерозе сосудов головного мозга у 10 больных из 12 была гиперпротромбинемия (106—123%). Нормальное содержание — у 1, пониженное также у 1 больного (86%).
При эндартериите сосудов головного мозга во всех случаях (кроме одного — 78%) протромбин крови был повышен (106—130,7%).
Наблюдая за динамикой протромбина при некоторых других поражениях нервной системы выявить какой-либо закономерности нам не удалось; незначительная гиперпротромбинемия в отдельных случаях сменялась гипопротромбинемией и наоборот, или же отмечалось нормальное содержание протромбина. Исключением были два больных опухолью головного мозга, у котовых обнаружена гиперпротромбинемия (111,8% и 123%).
В сочетании с общеклиническим обследованием больных, динамическое наблюдение за содержанием протромбина, особенно при анализе каждого случая гиперпротромбинемии,— весьма ценный объективный тест для своевременного профилактического назначения антикоагулянтной терапии.
В последние годы все большее число клиницистов становится сторонниками применения антикоагулянтов в профилактике и лечении тромбоэмболических поражений сосудов головного мозга. Б. П. Куше- левский, Д. Г. Шефер, Фоллей и другие сообщают о хорошем эффекте при лечении антикоагулянтами больных тромбозами и эмболией со
судов головного мозга, подчеркивая при этом профилактическое значение антикоагулянтной терапии.
Из 77 находящихся под нашим наблюдением подвергались лечению дикумарином 16 больных с поражением мозговых сосудов (тромбозом — 4, эмболией — 2, атеросклерозом — 2, эндартериитом — 5, другими поражениями головного мозга — 3).
Перед назначением дикумарина производилось клиническое исследование больных, исключались противопоказания, обязательно исследовалась моча на наличие эритроцитов и определялся уровень протромбина в крови.
Следует отметить, что лишь у 1 больного протромбин крови до назначения дикумарина был равен ICKF/o1, а у 15 чел. была гиперпротромбинемия (106,2— 125%), причем у 11—выраженная (свыше 111%). Эритроцитов в осадке мочи при микроскопическом исследовании не было обнаружено.
Дикумарин назначался по 0,1 в общей дозировке 0,6 в течение первых 3 дней, затем проводились анализ мочи, контроль протромбинового индекса, и, в зависимости от этих данных, назначалось дальнейшее лечение. После пробной дозы дикумарина (0,6) протромбин снизился в половине случаев на одну треть, а в другой половине—на две трети исходной величины, то есть был в пределах 40—74%. Максимальное снижение протромбина было до уровня 31,7%, каких-либо геморрагических явлений при этом не наблюдалось.
Суммарная доза дикумарина на курс лечения в наших примерах была в среднем 1,6. Дикумарин назначался на срок в среднем 12— 16 дней. У всех четырех больных с тромбозом сосудов головного мозга при гиперпротромбинемии (106—123%) после 10—12-дневного курса лечения дикумарином состояние улучшалось.
Для иллюстрации приводим выписку из истории болезни б-ного Л., 53 лет. Диагноз: Гипертоническая болезнь II ст. Тромбоз сосудов головного мозга.
Правосторонняя гемиплегия, сглаженность правой носогубной складки, язык отклоняется вправо. Мышечная сила в конечностях ничтожна, тонус понижен, справа— положительный симптом Бабинского. Протромбин—114,3%.
В течение 10 дней больной получил 1,3 дикумарина. Состояние больного улучшилось, стал передвигаться, явления гемипареза значительно уменьшились.
Положительный лечебный эффект от дикумаринотерапии достигнут при эмболии мозговых сосудов.
В качестве примера можно привести больную Д., 40 лет. Правосторонний гемипарез вследствие эмболии сосудов головного мозга, комбинированный митральный порок сердца. Положение в постели вынужденное — правосторонняя гемиплегия; долевая гемигипестезия. Правая носогубная складка сглажена, язык отклоняется вправо. Речь затруднена, была потеря сознания. Протромбин — 114.3%,. Проведено лечение дикумарином в течение 7 дней (1,0). В клинике больная находилась в течение месяца, движения в конечностях восстановились, начала ходить без посторонней помощи.
В двух случаях выраженного склероза сосудов головного мозга на фоне гиперпротромбинемии (ПО—111%) у больных в возрасте 62 и 72 лет применение йодистых препаратов заметного эффекта не дало, проведение же курса лечения дикумарином позволило добиться заметного улучшения, и больные выписаны в удовлетворительном состоянии: у одного явления левостороннего гемипареза значительно сгладились, а у другого, несмотря на повышение АД, удалось добиться хорошего субъективного состояния. У наблюдаемых нами больных склерозом сосудов головного мозга, не леченных дикумарином, такого выраженного лечебного эффекта не наступило.
Мы считаем, что назначение дикумарина при склерозе мозговых сосудов на фоне гиперпротромбинемии оправдано и имеет не только лечебное, но и профилактическое значение.
Выраженный лечебный эффект достигнут при назначении дикумарина во всех 5 случаях эндартериита сосудов головного мозга. У всех
были выраженная гиперпротромбинемия (111 —125%), ограничение движений в конечностях, ослабление мышечной силы и явления геми- гипестезии болевой чувствительности, у некоторых — патологические сухожильные рефлексы, нарушение речи, головная боль. На курс ле- чения больные получали 1,6—6,0 дикумарина, в ряде случаев назначались физиотерапевтические процедуры и лечебная гимнастика.
В качестве примера приводим следующие две выписки из истории болезни.
Б-ной К., 50 лет. Правосторонний гемипарез с моторной афазией вследствие энд- артериита. Протромбин — 111%. В течение 18 дней получил 1,6 дикумарина. Появились движения в ноге, улучшилась речь, б-ной выписан.
Б-ной Б., 56 лет. Специфический эндартериит сосудов головного мозга, эндарте- риит нижних конечностей. Протромбин—118,8%. В течение 26 дней получил 2,4 дикумарина. Состояние больного постепенно улучшалось, сила в правой руке значительно восстановилась, речь стала свободной.
В итоге проведения лечения из 77 больных у 49 состояние улучшилось, у 18 видимых изменений не отмечено, у 10 наступил смертельный исход (у 5 — с кровоизлиянием в мозг и у 5 — с тромбозом мозговых сосудов),,
ВЫВОДЫ
- Наблюдение за содержанием протромбина крови при сосудистых заболеваниях головного мозга важно для диагностики, прогноза, а также для контроля за течением заболевания и своевременного назначения антикоагулянтной терапии.
- Дифференциальная диагностика различных форм сосудистых поражений головного мозга трудна; гиперпротромбинемия и лабиль-* ность уровня протромбина в крови у больных геморрагиями и тромбозами сосудов головного мозга являются неблагоприятным симптомом в отношении исхода заболевания.
Клинические наблюдения подтверждают целесообразность антикоагулянтной терапии при тромбоэмболических поражениях сосудов головного мозга.

Adverse reactions and complications when using new drugs

There are a number of classifications of side effects and complications in the use of drugs, including Soviet authors (E. M. Tareev, A. F. Bilibin and K-V. Bunin, Sh.D. Moshkovsky, XX Planelles, A. L. Libov , P. L. Seltsovskii and B. S. Briskin and others). However, these classifications are built taking into account the side effects of chemotherapy drugs, primarily antibiotics.
