Organization of anesthesia service in a rural district hospital

Abstract

The issue of anesthesia during surgical and other operations was solved quite recently in almost all rural hospitals, mainly by the use of local anesthesia according to the method of A.V. Vishnevsky. The number and nature of the operations were quite consistent with this method. According to prof. VI Struchkov, local anesthesia was used in 72.52% of all operations performed in the USSR in 1951-52. Only sometimes there was a need for anesthesia, and in this case, any of the doctors or even any nurse (paramedic) was given a simple ether anesthesia. Modern methods of anesthesia were not used because of the novelty and seeming cumbersomeness of the method, which frightens off rural surgeons.

About ruptured uterus

Abstract

For 5 years (1954-1958) in maternity hospitals in Kazan, uterine ruptures were observed in 29 women. At the age from 20 to 30 years old there were 10, from 30 to 40 years old - 17, over 40 years old - 2 women. There was only one primary pregnant woman, multiparous and multiparous 28. Ruptures were observed at least at 2-4, maximum at 10-11th pregnancies. In 15 patients in the past there was a caesarean section, in 13 obstetric history was aggravated by either febrile, artificial, more often criminal abortions, or prolonged labor, febrile postpartum period, which probably caused destructive changes in the muscles of the uterus and led to its functional impairment in subsequent childbirth.

We studied fertility in women who had previously suffered from ascending gonorrhea and were treated in hospitals of the Venereal Diseases Institute and the city vendispensary in Gorky. When identifying infertility, we tried to find its cause. For this purpose, in a number of patients we performed hysterosalpingography, urine examination for pregnandiol in the secretory phase (to identify anovulatory cycles) and in some patients - the Shuvarsky test (for the compatibility of cervical secretions and spermatozoa).

In some diseases of the chest organs, local neurological disorders can occur. In particular, in the clinic of lung diseases, the pain phenomenon sometimes accompanies pathological processes in the pleural sheets. More complex neurological manifestations in the clinical picture of the disease of the lungs and mediastinum occur when the process is localized in the medial parts of the lungs with adjacent mediastinal parts of the pleura, in the apex of the lung, with subsequent changes in the apical pleura, in the costal-vertebral corners, in the immediate proximity to the diaphragm. In these complex anatomical zones, in close proximity to them, there are neuro-vegetative nodes, powerful sympathetic and parasympathetic plexuses, the trunks of the phrenic and vagus nerves pass. As a result, in a number of cases, pathological processes, varied in clinical forms, depending on the location and extent of anatomical changes, cause a complex complex of neurological symptoms that is unique for each case.
