Use of drugs of pyrimidine series for the treatment of experimental immune heart lesion

Abstract

By means of immunization by the mouse heart homogenate and streptococcic allergen on guinea-pigs the model of the pathologic process is reproduced where the changes of hematologic and immunologic blood indices, the clinical picture of the heart lesion are similar to the changes observed in patients with rheumatism. The use of drugs of pyrimidine series such as methyluracil and xymedone benefits the positive dynamics of blood indices and the decrease of the manifestation of pathologic changes in the heart. Xymedont is superior to methyluracil in efficacy allowind to of patients with rheumatism.

New antibiotic tienam in the treatment of children with severe forms of sepsis

The intimate direct correlation between poll- resistance to antibiotics and the evidence of microbe pathogenicity as hemolytic fibrinolytic activity and chromogenesis is revealed.

Ligation of the bronchus in the base in pneumonectomy

Abstract

The circular ligation of the bronchus stump in the base without lumen suture retaining the nourishing graft along the back semicircle is proposed. The bronchus stump formed by the method proposed endures the endobronchial pressure more than 1 atm. In aseptic conditions the adhesion follows the pattern seen in first intention retaining the blood supply of the terminal part for all terms after the operation. The rate decrease of postoperative complications in the stump is determined by the exception of sputum congestion and infection penetration along the suture channel, the provision of the optimal contact of the mucosal edges and its primary adhesion in short terms and prolonged reliable hermeticity, coservation.

Immediate results oe the programmed treatment of patients with acute leukemia

Abstract

The pronounced progress in the achievement of full remission (in acute myeloid leukemia— 58,8%, in acute lymphoblastic leukemia— 66%) is revealed in studies of the results of randomizational use of intensive programs of polychemotherapy of 23 patients with acute leukemia. The intensification of the programmed treatment is connected with a high risk of complications. For their decrease the adequate maintaining therapy and the treatment in aseptic wards become necessary.

One of the urgent problems of neurosurgery and endocrinology is the early diagnosis of pituitary tumors, which rank third among the neoplasms of the nervous system [1]. Patients with these tumors account for 13.3% of all patients with neurooncological pathology, about 75% of them are people aged 20 to 50 years [1]. On the territory of Russia, the annual increase in the number of patients with pituitary adenomas reaches 3000, while 80-100 thousand patients need treatment or active observation [3].

Role of irradiation therapy in the treatment of malignant tumors of soft tissue

As many as 254 patients with sarcomas of soft tissue were under observation for 5 years. The classification of malignant soft tissue tumors depending on their irradiation sevsitivity is developed. The data for the rate of local relapses and romote metastases in histologic variants of tumors and the data for five-years survival without relapses are presented. Based on the irradiation sensitivity of sarcomas of soft tissue the treatment methods are recommended.

Abstract

The relevance of the research of border mental disorders (PPR) after radical oncological operations is determined by their rather large prevalence. The frequency of border mental disorders, according to different authors, varies from 26 to 92% [2, 4, 5], which depends on the personality constitutional features, the phase of the disease, the effectiveness of its therapy. It is believed that the "gravity" of the oncological diagnosis is the strongest psychotrauming factor along with such as the radicality of the operational intervention about the malignant neoplasms, the need for pre-postoperative coursework radiation and (or) chemotherapy.

Reasons and prevention of repeat operations in dupuytren’s contracture of fingers

Abstract

Based on the results of the operative treatment of patients with Dupuytren’s contracture of fingers (more than 1000 operations) in general surgical and traumatologic departments of hospitals, as well as in the specialized department, it is established that the most often reason for the repeat operations is deficient skill of surgeons performing the first intervention. It is appropriate to reveal Dupuytren’s contracture at the beginning when the operative intervention is more easily fulfilled. The operative intervention is more efficient in the specialized department.

Morphofunctional changes in uterus tissue in the laser «welding»

Abstract

The morphofunctional changes in. uterus tissue in laser «welding» suture and in reparation process stages in correlation with traditional surgical suture are studied in acute and chronic experiments on dogs and rats. It is established that in laser «welding» suture 4 zones of microcirculation disturbance: tissue and microvessels coagulation, stasis, reactive-destructive and disfunctional changes of microcirculatory channel are formed in uterus tissue. The recovery of the microcirculatory channel in laser suture occurs earlier in correlation with the traditional one. The data of the manometric method show the high mechanic strength of laser «welding» suture.

Abstract

The degree of atherosclerosis of pancreatijjgt arteries and the condition of insular аррагщг tus of the pancreas in 86 elderly persons with various clinical types of diabetes and without it are studied. More severe atherosclerotic damage of chall pancreatic arteries in diabetes is shown. The direct dependence of the degree of atherosclerosis on the long persistence of diabetic anamneses is revealed only in type II diabetes, in type I diabetes the dependence is opposite. It is considered to be due to the onset of type I diabetes in eldarly patients, with principally regional atherosclerosis damage of pancreatic arteries. In the diagnosis of type II diabetes the sharp distinction of islets diameters and fibrosis nature are noteworthy.

The use of computed tomography in clinical practice

Abstract

The increasing role of radiation diagnostics in clinical practice is due to the constant improvement of equipment, improvement of the quality of the images obtained, and more rational use of traditional methods of examining patients. Radically new types of image acquisition and processing include X-ray computed tomography (CT), in which an X-ray tube connected to a data collection system rotates in an axial plane around the patient's body. The narrow X-ray beam emitted by the tube with a width of 1 to 10 mm, passing at different angles through the studied layer at each point, weakens to a different degree, which is captured by the detectors, numerically displayed in the data acquisition system and then processed by a fast processor using the method of back projections [11]. All transverse images obtained in numerical form are fed to the "main" matrix of the computer.

Increasing industrialization and concentration of industries in large cities lead to significant pollution of atmospheric air, soil and water with harmful substances. The most powerful sources of pollution are chemical agents emitted by industrial enterprises, as well as vehicle exhaust gases containing more than 200 chemical compounds.

Abstract

The scientific session of the Cardiological Scientific Center of the Russian Academy of Medical Sciences was devoted to ischemic heart disease. Acad. LAMN, prof. VN Smirnov in the message Atherosclerosis and Ischemic Heart Disease: Interrelation of Development Mechanisms "noted that now no one doubts the role of cholesterol in the development of ischemic heart disease. According to Swedish scientists, lowering cholesterol levels by 30% reduces mortality from myocardial infarction (MI) by the same amount. As the speaker noted, today human health must be considered from the point of view of the state of his vessels. He presented, as it were, a revived theory of the inflammatory origin of atherosclerosis, the essence of which is damage to the vascular endothelium by a nonspecific inflammatory agent. If in a newborn the endothelium has a fine-mesh structure, then by 3-5 years large endothelial cells, the so-called clusters, appear in this finely branched network. By the age of 20, the surface of the vessels, on which mainly atherosclerotic changes are detected, for example, the large aorta, becomes heterogeneous.
