Abstract

On the traditional day of the Kazan Institute for Advanced Training of Physicians, I had the great honor to give an act speech in front of its staff. It was based on the results of scientific works of the staff of the department in 1981-1986 mainly on studying the role of biogenic amines and some hormones (prolactin, hormones of adrenal cortex, parathyroid hormone and calcitonin) in the pathogenesis of some neuroendocrine diseases, which occur in obstetric-gynecological practice.

The use of computer technology makes it possible to increase diagnostic capabilities of traditionally used methods of cardiovascular system investigation and significantly reduce the examination time. Methods of automatic processing of ECG, sphygmograms and rheograms are most developed. The aim of this work is to use computer processing of apexcardiograms (ACG) to develop a set of quantitative indices characterizing mechanical activity of myocardium.

Temporal and amplitude parameters of systole and diastole in dilated cardiomyopathy

Abstract

The aim of the present investigation was to study left ventricular pelimechanocardiogram (ECG, FKG, left ventricular cardiogram, sphygmogram) and estimate their changes in dilated cardiomyopathy. We studied 40 patients with dilated cardiomyopathy aged from 28 to 58 years (mean age, 43.7 years). There were 34 men and 6 women. The diagnosis of cardiomyopathy in all patients was verified by echocardiography and in 5 deceased patients was confirmed by sectional study. The control group consisted of 20 healthy persons aged 16 to 55 years.

Diagnosis of anterior branch block of the left Gis bundle by orthogonal ECG

Abstract

The aim of our work was a comparative study of ECG signs of block in the corrected orthogonal leads of the Franck system with reversed polarity, in the lead and in the usual 12 leads. We studied 150 healthy people and 70 patients (39 males, 31 females) with left bundle branch anterior blockade. There was 1 patient under 20 years of age, 21 to 30-2, 31 to 40-10, 41 to 50-13, 51 to 60-16, and 28 over 60 years of age.

Changes in central and regional blood circulation in patients with coronary heart disease under the influence of nitro ointment

The aim of the study was to investigate changes in central and regional blood circulation occurring after a single application of 2% nitro ointment produced by Orion Pharmaceuticals (Finland) in patients with coronary heart disease (CHD). We have studied 39 patients aged from 31 till 72 years (mean age - 54,4 years) with angina pectoris of tension and rest (angina pectoris frequency - not less than 7 attacks a week) of II-IV functional class (according to classification of All-Russian Center of Medical Science of the USSR).

Study of acoustic cardiac symptomatology in patients with acute myocardial infarction

The main objective of this study was to study I and II heart tones using phono- and spectrography in healthy subjects and in patients with acute myocardial infarction. We examined 79 patients (59 men and 20 women) with acute transmural large focal myocardial infarction aged 30 to 77 years. The diagnosis of acute myocardial infarction was made on the basis of clinical, electrocardiographic and laboratory studies.

The use of paracoagulation tests to control heparin therapy in patients with acute myocardial infarction

Abstract

The aim of the work was to determine the optimal doses of heparin for the prevention of thrombotic complications in patients with acute myocardial infarction based on the results of paracoagulation tests. We followed 269 patients with acute large focal and transmural myocardial infarction, verified by typical development and course of the disease, characteristic ECG dynamics in 12 standard leads, in some cases supplemented with three corrected orthogonal leads in Frank's system, blood picture dynamics and typical hyperenzymemia. In 49 cases pathological and anatomical investigations were carried out.

Prognostic value of cardiometry in myocardial infarction

Abstract

We tried to find out the influence of absolute heart size in patients in the acute period of myocardial infarction on long-term life and labor prognosis. For this purpose we studied long-term results (in 4-5 years) in 127 patients with myocardial infarction, who were treated at the specialized department of "Izhmash" hospital. All these patients in the wards of intensive care, along with other clinical and laboratory tests, on the first day of stay in the hospital trochography with cardiometry was carried out with a portable X-ray unit of P5 brand at focal distance of 1 m, voltage 83 kV, current strength 150 mA and exposure time 0,1 s.

Assessment of scarring time in myocardial infarction

Abstract

The aim of this work was to study the concentration of collagen metabolite peptide-bound hydroxyproline in the dynamic observation of patients with myocardial infarction. We observed 80 patients, including 50 patients (31 men and 19 women) with myocardial infarction aged 34-72 years. In 7 patients the infarction was small focal, in 26 patients it was non-transmural, and in 17 patients, it was transmural.

Morphological characteristics of the thymus gland in children with congenital heart disease

We have undertaken a study of thymus gland structure in children with congenital heart defects aged 3 to 10 years. We analyzed thymus biopsy specimens obtained from the marginal zone of the gland during surgical correction of cardiac malformations after thoracic cavity dissection. The biopsy size was 1.5 cm3.

Abstract

We studied microcirculation and capillary permeability in 70 patients with nephrogenic hypertension and 40 healthy subjects. The main contingent of patients was under 40 years old. Bulbar biomicroscopy was used to investigate the state of the microcirculatory bed by means of slit lamp SLT-U 4.2. To describe the state of microcirculation we used quantitative-qualitative assessment, intravasal status was determined according to the method of V.F. Bogoyavlensky.

Diagnosis of renal dysfunction in hypertensive patients using the ß2-microglobulin test

Abstract

The aim of the present work was to study the clinical value of ß2-microglobulin test to detect renal disorders in hypertensive patients. The ß2-microglobulin test was performed in 118 people, including 88 hypertensive patients (53 women and 25 men), 18 men with chronic hypertensive glomerulonephritis, and 12 healthy individuals. The age of the patients ranged from 26 to 59 years. Depending on the stage of the underlying disease, the patients were subdivided as follows: 19 patients were verified to have stage I hypertension, 42 patients had stage II, and 27 patients had stage III.

Clinical variants of vasculogenic impotence

We examined 53 patients suffering from obliterating atherosclerosis with localization only in the aortoiliac segment, who were treated in the clinic from 1982 to 1984. The patients were aged from 42 to 61 years. According to the WHO classification, 16 (24.5%) were young (under 45 years) and 37 (75.5%) were middle-aged (46-60 years).

Prediction of postpartum purulent-septic complications using intradermal tests

Abstract

Intradermal tests for functional assessment of T-cell immunity were performed on 95 women who gave birth. There were 39 women aged 19 to 24 years, 25 to 29 to 36, and 30 to 35 to 20. Bacterial allergens were injected at a dose of 0.1 ml on the inner surface of the forearm strictly intradermally at 7-8 cm apart. The results were evaluated after 24 h. The reaction was considered positive if the size of the infiltrate was equal or greater than 8 mm.

Prevention of disruption of adaptation in children upon entering preschool and school

Abstract

In order to provide a pathogenetic substantiation and develop a system of measures to facilitate children's adaptation to new microsocial conditions, a comprehensive dynamic examination with a unified methodological approach was conducted in 299 children of early childhood, preschool and primary school age on entering nursery, kindergarten and school. The initial state of health, the level of physical and mental development, and, in addition, the degree of psychofunctional readiness for learning were examined in all children. The data of the social and biological anamnesis were analyzed in detail.

Using genealogical analysis to identify children at increased risk of diabetes mellitus

In order to identify risk groups, genealogical analysis was performed on the pedigrees of 711 children who were on the dispensary registry in children's endocrinology offices in Saratov for various endocrine disorders. In addition, together with pediatric endocrinologists we conducted a targeted medical examination of 11978 children in 41 preschool institutions and 5 general education schools according to the screening program with the help of Glucotest.

Radiological features of hypertension-hydrocephalus syndrome

Abstract

The aim of this work was to differentiate the signs of hypertension and hydrocephalus, analyze and systematize them. We carried out complex radiological study of 97 patients (67 males and 30 females) with hypertension and hydrocephalus syndrome aged 25-30 years old with the determination of pressure in the liquor system. Observations showed that hypertension (increase of cerebrospinal pressure) in 17 patients proceeded without hydrocephalus.

The effect of isoptin on sinus arrhythmias arising from hypoxia

It was found that exposure to isoptin reduced the amplitude of action potentials by 15.8 mV (41.19%), the pre-potential value by 11.5 mV (68.6%); prolongs the phase of slow diastolic depolarization by 176.5 ms (54.5%), the phase of systolic depolarization by 20.12 ms (29.1%); shortens repolarization by 34.5 ms (17.5%) and slows the frequency of spontaneous activity by 22.0 per 1 min (18.4%).

In the available literature we have not found works on the study of cationic proteins in acute myocardial infarction. In this regard, we set a goal to identify the level of neutrophil cationic proteins in patients with acute myocardial infarction, to determine the sensitivity of this index and its dynamics in relation to the generally recognized laboratory tests (CRP, degree of leukocytosis), to assess the value of the test as an additional laboratory criterion, characterizing necrotic and resorptive processes in the dynamics.

The pathogenesis of obliterating endarteritis of the lower extremities

Abstract

We analyzed the case histories of 772 patients with obliterative endarteritis of the lower extremities. The collected material indicates that both direct and reflex effect of etiological factors on the fingers of the lower extremities is important in the genesis of this disease. Direct influence of trauma, low temperature, infection, vibration and heavy load was in 533 (69%) patients, and reflex influence of nicotine and other neurotoxins, trauma, low temperature, arthroso-arthritis, spine osteochondrosis and other pathological processes in topographically distant from fingers areas and organs as well as nervous and emotional shocks and mental stresses was in 239 (31%).

Long-term results of treatment of patients with limb arterial embolism

Abstract

We followed the fate of 166 patients with limb arterial embolism in the late stage of the disease after conservative and surgical treatment from 6 months to 10 years. Of 166 patients, 44 (26.5%) died after hospital discharge. Thirty of them were older than 61 years. The most frequent causes of death in the long-term period were acute cerebrovascular disease (11), myocardial infarction (7), progressive cardiovascular insufficiency due to cardiac defects (6), repeated peripheral artery embolism causing limb gangrene (7). In 7 patients the cause of death was not determined.

Microbial allergy in patients with chronic destructive pulmonary tuberculosis

Abstract

The aim of our work was to detect microbial allergy in patients with chronic destructive pulmonary tuberculosis. We followed up 114 patients, 14,9% of them were under 40 years old, 41 to 50-24,5%, 51 to 60-38,7%, 21,9% were aged over 61. The duration of TB disease up to 5 years was ascertained in 20.6% of examined patients, from 5 to 10 years - in 26.4%, from 10 to 20 years - in 24.5% and more than 21 years - in 28.5%.
