Васильев на 63 случая паховой грыжи левостороннее положение червеобразного отростка встретил у 5 детей. Острый аппендицит в паховой грыже описан Б. Н. Назаровым (Хирургия, 1955, 2). С. Е. Лабинский приводит случай ущемленной грыжи в сочетании с деструктивным аппендицитом (Хирургия, 1957, приложение к журналу).
В клинику госпитальной хирургии Куйбышевского медицинского института 17/VI-57 г. в 21.00 для оказания неотложной хирургической помощи поступил ребенок Гасилин Анатолий, 6 месяцев, с явлениями, со слов матери, болей в животе и припухлости в области мошонки слева.
Заболел 16/VI-57 г; в 18.00 внезапно появились резкие боли в левой паховой области. Выпячивание, которое раньше вправлялось, приобрело плотную консистенцию и в брюшную полость не вправлялось. Боль усиливалась с каждым часом, но за медицинской помощью родители не обращались. 17/VI-57 г. боли усилились, появилась рвота. В связи с ухудшением состояния ребенок был доставлен в клинику. Грыжевое выпячивание появилось 3 месяца назад И свободно вправлялось в брюшную полость. Общее состояние ребенка при поступлении в клинику удовлетворительное. Пульс — 92 удара в мин, ритмичный. Язык чистый. Живот симметричен. Стул — 4 раза в сутки. Мочеиспускание свободное, безболезненное. Правое яичко не изменено. Левое яичко четко определяется и находится рядом с грыжевым выпячиванием, которое располагается в левой пахово-мошоночной области, имеет овальную форму, размер 8 X 5 см, почти безболезненно. Начинаясь у наружного отверстия пахового канала, грыжевое выпячивание опускается в мошонку и достигает верхнего полюса яичка. Воспалительных изменений нет. Над выпячиванием наблюдается тимпанит. Образование в брюшную полость не вправляется.
Произведено срочное оперативное вмешательство. В грыжевом мешке оказалась слепая кишка с червеобразным отростком. Цианотичная слепая кишка приобрела нормальный цвет. Червеобразный отросток синюшный, длиной 8 см, напряженный. Произведена типичная аппендэктомия. Грыжевой мешок ушит изнутри. Остатки его иссечены.
Пластика пахового канала, по Жирару. Швы на кожу.
Описание препарата. Отросток длиной 8 см, при разрезе слизистая отечная, с множественными кровоизлияниями, местами темнобагрового цвета, в просвете — каловый камень.
Послеоперационный период протекал без осложнений.
