Andreicheva E.A., Andreichev N.A.

Abstract

The amplitude indices of QRS complex are studied by R.Z. Amirov’s integral topography with the record of 104 Vilson’s monopolar leads from the chest surface in 42 persons in norm depending on the heart electric axis position as a control group and in 47 patients with the blockade of the anterosuperior ramus of His0 bundle left crus. The results obtained allow to derive a comprehensive idea of the electric field character in the blockade involved and to explain the variability of electrocardiographic picture in chest leads of classic ECG. The additional criterion of the blockade of the anterosuperior ramus diagnisis with high sensitivity and specificity is proposed.

Oxygen metabolism of polymorphonuclear leukocytes of peripheric blood as a criterion of gestosis treatment efficacy

Bashkirova D.S., Safina N.A., Zinkevich O.D.

Abstract

The effect of gestosis pathogenetic therapy on oxygen metabolism of polymorphonuclear leukocytes of peripheric blood is studied. Enterosorbents, cell membranes stabilizer and eubiotics are used as therapeutic agents. The partial normalization of oxygen metabolism of leukocytes: the decrease of spontaneous chemoluminescence by 50—70% in the presence of treatment and induced chemoluminescence by 20—25% in comparison with initial level before treatment are shown. The direct correlation of this characteristic with clinical picture shows the possibility of its use as an objective laboratory criterion of the therapy efficacy.

Sinuses structure of lymphatic nodes in norm and in metastatic spreading process of cancer

Tsyplakov D.E., Shamsutdinov N.S.

Abstract

The sinuses of regional lymphatic nodes obtained during operations of 50 oncologic patients are died using ultrastructural and morphometric method. The investigations are performed in 3 groups unow observation: 1) control (lymphatic nodes of practically healthy persons being lost from casual circumstances), 2) lymphatic nodes without metastases, 3) lymphatic nodes with metastases of various volume. It is established that the pronounced macrophagal and T-cellular reactions retard metastatic spreading in early stages of tumoral growth (I, II stade of cancer more rarely). The intensity of these reactions decreases noticeably providing holding in sinuses of malignant cells and generalization of tumoral process.

Results of an experiment in preferential observation of young children in the outpatient clinic

Purtov I.I., Albitsky V.Y., Fedorovich L.A.

Abstract

The existing system of medical care for children does not allow increasing the volume and improving the quality of the district doctor's work in raising a healthy child, despite the availability in each territorial children's clinic of specially created and equipped rooms (rooms) for healthy children and dedicated nursing staff for this purpose. An analysis of the work of the district pediatrician, carried out by employees of the Gorky Research Institute of Pediatrics of the RF Ministry of Health, showed that the doctor has to spend the bulk of his working time treating sick children. Healthy children are treated by a doctor for only 27.4% of his working time. During periods of increased morbidity, the amount of work with healthy children is reduced to a minimum.

Treatment of chronic gastroduodenal ulcers associated with Helicobacter Pylori-infection

Soshin A.N.

Abstract

Treatment of chronic gastroduodenal ulcers remains one of the most difficult and unsolved problems. The main difficulty that they try to overcome is their recurrent course. Nowadays the main directions of medical therapy of peptic ulcer disease are the decrease of intragastric acidity and sanation of gastroduodenal mucosa from Helicobacter Pylori. Treatment aimed at the destruction of Helicobacter Pylori allows to reduce the relapse rate to almost zero, provided it is successfully suppressed.

Abstract

Treatment of patients with acute pancreonecrosis is very difficult, which is associated, in addition, with a change in the physical and chemical properties of the plasma. The latter becomes viscous, white in color, sometimes with a grayish or dirty tint. Such plasma blocks microvascular bed with the development of respiratory distress syndrome, hepatic and renal failure. Common method of forced plasmapheresis with exfusion of 1.2-1.5 liters of plasma is not always effective.

Hand oleoma

Nettov G.G.

Abstract

Oleoma (synonyms: lipogranuloma, oleoscleroma, lipoid granuloma, etc.) is a focus of chronic inflammation characterized by the presence of small cysts lined by large cells containing fat and giant multinucleated cells in the infiltrate. Oleoma usually occurs at the site of injection of oily solutions of drugs. The usual places of localization of injected granulomas (oleomas) are, as a rule, the gluteal area, the outer surface of the shoulder, and the scapular area. Our clinical observations show that the cause of oleoma formation is the injection of liquid vaseline, and in an unusual place - in the tissues of the hand (2 cases - three hands).
