The phenomenology of psychic disorders developing in visitors of "mass psychotherapy" performances is described. The proposals on pathogenesis of these disorders are made. The method of the emergency service in these situations — stress psychotherapy using dosed hypoxia, imperative suggestions and punctate massage methods is described.

Algesic syndrome in the epigastric region in extraabdominal pathology, its place in intensive diagnosis

Abstract

As many as 98 patients with extraabdominal pathology having pain in epigastric region as a basic symptom are examined. Among them 10 patients with neurogenetic abdominalgic syndrome and 8 patients with masked depression received due attention. The careful clinical examination before instrumentation, comprehensive anamnesis, algesic syndrome peculiarities are basic in the diagnosis, because the mechanism of its origin becomes clear at this stage. The use of uneffective, nonresultant investigation methods is excluded, the time and material expense for examination are reduced.

Surgical treatment of colonoptosis in children

Abstract

The results of the treatment of 126 children with colonoptosis aged 7 to 14 are presented. As many as 45 children were operated. The operation was performed using the method of creating ligaments from peritoneal grafts and fixation of hepatic and splenic angles of the colon to them. The isolated colonopexia was performed in 7 patients (15,6%). In 38 patients (84,4%) colonopexia was supplemented by simultaneous operations: nephropexy, gastropexy, duodenopexy, cecopexy, appendectomy. The remote results at the period from 6 months to 5 years are followed in 40 patients. In 95% of the observations the results of the operations are considered to be good.

The comparision of two diagnostic methods for detection of Helicobacter pylori in gastric mucosa

Abstract

The two diagnostic methods for detection of Helicobacter pylori have been tested. The first one was de-nol test, which was based on detection of the urease activity. The second one was the immunocytochemical method using the polyclonal antibodies against Helicobacter pylori. During the fibrogastroscopy of patients with gastric desease (20 persons) the small pieces of gastric mucosa were taken. The latters were used for preparation of smears and then for de-nol test. 18 patients have got the positive de-nol tests, and the analyses of two patients were negative. However, the Helicobacter pylori were detected in all cases, using the immunocytochemical staining with polyclonal antibodies against this bacterium. On the base of these results we have suggested that immunocytochemical staining of Helicobacter pylori is more informative and more precise than de-nol test.

Efficiency of the use of kwamatel for the treatment of duodenal ulcer

Abstract

The clinical efficiency of kwamatel in the treatment of the relapse of duodenal ulcer, expressed in the decrease of night painful syndrome to the 3d day of the treatment, day painful syndrome to the 4—5 th day, cicatrization to the 28 th day (in 97%) is confirmed. The examination of patients in 3 months and a year revealed the decrease of subjective manifestations of duodenal ulcer in 86,9% of the patients, the initiation of the early relapse in 13% of the patients, the relapse of duodenal ulcer throughout the year in 34,8% of the patients.

Apparatus method in the treatment of eventration into the purulent wound

Abstract

The clinical use of the apparatus method in 15 patients with eventration into the purulent wound allows to obtain the best immediate and remote results comparing with the traditional method. The reliable closure of the peritoneal cavity is achieved during the junction of the wound lips. The junction of the wound li ps by stages occurs without destruction of tissue and the optimum conditions are created for the course of regenerative processes.

New method of the application of choledochoenteroanastomosis with protection

The new method of the formation of biliodigestive anastomosis without the basic defect of existing methods of the surgical intervention — regurgitatior of enteric contents with the following development of ascending cholengitis is proposed. The type of anastomosis proposed is recommended for the approbation in clinical conditions for reconstructive operations of biliary tracts.

Primary generalized amyloidosis with preferable lesion of cardiovascular system and lungs

Abstract

The rare case of the fast development of amyloid cardiomyopaty in initial amyloidosis is described. The generalized amyloid lesion of vessels (especially the lesion of the heart and lungs), prolonged use of steroid therapy accelerating amyloidogenesis, incompetence of immune reactions promoting amyloid resorption inhibition served the great purpose in the gravity of the disease.

Anticoagulant and antiprotease therapy of patients with pulmobronchopleural complications of surgical sepsis

Abstract

The state of hemostasis in 152 patients with bronchopulmonary complications of surgical sepsis is investigated. The thrombohemorrhagic syndrome is diagnosed wherein anticoagulant therapy with heparin is prescribed. Antienzymic therapy using such protease inhibitors as contrical, hordox and E-aminohexoic acid is performed in 48 patients. The terms of normalization depending on the gravity of the pathologic process comprise from 4 to 9 days.

Treatment of injuries of forearm peripheric nerves by the distraction method

Abstract

On the basis of the study of the results of distractional stretching of peripheric nerves, and as well as their comparison with the results of the treatment using plaster bars, a number of advantages of the apparatus method is revealed in replacing nerves defects. These advantages allow to improve significantly the results of the treatment of patients concerning the recovery of sensory and motor functions of the injuried peripheric nerve.

Use of gel with prostaglandin e2 in the context of programmed labors in primiparas

Abstract

The labor characteristics and contractile activity of the uterus resulting from programmed labors after the intracervical application of gel with prostaglandin E2 are studied. The gel with prostaglandin E2 was not used in the matched control group of patients with programmed labors. After introduction of the gel the modified Bishop scores significantly improved, the labor began in one of the patients. During introduction of the gel the hypertonus of the uterus or the pathology of the fetus heart rate is not observed. The length of the active phase and the second period of the labor is significantly shorter than in the control group. The contractile activity of the uterus is similar in both groups. The primary cervical effect of the gel is supposed.

Study of the functional state of the only uterine tube in women after the tubal pregnancy operation

Abstract

The state of the second remained tube after the- tubal pregnancy operation in 16 patients is studied by kymographic tubal insufflation. Salpingectomy is performed in 14 patients and linear salpingotomy of the only uterine tube is performed in 2 patients. The other tube is removed during the first tubal pregnancy operation. It is found that in all patients the remained tubes are permeable as this takes place permeability, contractile activity and contraction rate decrease in 7 patients (in 5 patients after salpingectomy, in 2 patients after salpingotomy). Kymographic tubal insufflation is the information investigation method of the state of the remained uterine tube after the tubal pregnancy operation.

Acupuncture and manial therapy in the treatment of neuroendocrinal syndromes in gynecology

Abstract

The experience of the treatment of neuroendocrinal syndromes by traditional methods: acupunctural therapy and in combination with physiotherapy or with manual therapy is presented. Little information on each pathology, methods of acupunctural and manual therapy is given. K. Akabane test is chosen for diagnosis and treatment. For the estimation of the efficiency of acupunctural therapy the functional diagnosis tests, contents study of hormones, catecholamines, serotonin and biologically active substances are used. The favourable clinical effect in each group of patients supported by laboratory data suggesting the increase of reserve possibilities of the functional activity of hypothalamohypophysial and gonadal system is obtained.

Change of peripheric blood indices in patients with inflammatory genesis sterility after manual therapy

Abstract

The original treatment of women with tuboperitoneal sterility in the presence of adhesive process in the small pelvis and abdominal cavity owing to chronic inflammatory diseases of genital organs is performed by manual therapy methods. The dynamic control of peripheric blood indices reliably showed the positive dynamics of the number of lymphocytes and monocytes. Pregnancy occurred in 19,5% of the patients.

Target organs in arterial hypertension

Abstract

One of the consequences of long-term BP increase is damage of internal organs, the so-called target organs (target organ damage, end organ damage). These include heart, brain, kidneys, vessels. Heart damage in arterial hypertension may be manifested by left ventricular hypertrophy, angina pectoris, myocardial infarction, chronic heart failure and sudden death, brain damage - by thrombosis and hemorrhages, hypertensive encephalopathy and cerebral lacunae, kidneys - microalbuminuria, proteinuria, chronic renal failure, vascular involvement - lesions of retinal vessels, carotid arteries, aorta (aneurysm).

Esophageal diverticular cancer

Abstract

One of the serious complications of esophageal diverticula is the development of cancer in it. In epiphrenal diverticula malignization is more frequent than in bifurcation diverticula. On the basis of statistical data analysis I.T. Shevchenko (1950) established that tumor in pulsatile diverticulum occurs in 1.2% of cases and in traction one - in 7.8%. In his opinion, one of the causes of esophageal tumor is a tractional diverticulum.

Quantitative estimation of the effect of noxious substances in atmosphere air on the sickness rate of children

The sikness rate of children living in the territories of the city with various levels of anthropogenic load is studied. The informativity of the multifactor dispersed analysis for determination of quantitative characteristics of the effect of environmental factors on the population sickness rate is noted. The effect of six inassociated factors (dust, sulphur dioxide etc.) on the sickness rate of children aged 4 to 7 is studied by this method.

Stages of the formation of clinical anatomy in Russia

The history of clinical anatomy is quite complex. The progression of any fundamental discipline, and anatomy in particular, is reflected primarily in its content, which is put into concrete form. Born during the Renaissance, anatomy predetermined the success of medicine. The use of the deductive method in the practice of medicine led to the creation of descriptive anatomy, and of inductive anatomy to surgical anatomy with the condition that it must be taught to surgeons.

Calcium channel blockers and carcinogenesis in the elderly

Lancet August 24, 1996 published an article by M. Pahor, J.M. Guralnick, L. Ferrucci, M.-C. Corti, M.E. Salive, J.R. Cerhan, R.B. Wallace, R.J. Havlik from different universities and medical institutions of Italy and the USA, where the problem of carcinogenesis risk as the result of widely used calcium channel blockers (CCBs) in treatment of hypertension is discussed. The editorial board of the Kazan Medical Journal, due to the importance of this problem, decided to introduce the main points of this article to its readers.
