Correction of hemodynamic disorders in patients with ischemic heart disease complicated by intraventricular blockades

Abstract

The effect of beta-adrenergic drugs on central and peripheric hemodynamics in patients with ischemic heart disease with blockades of His'bundle peduncles in doses for one occasion is studied. Neoepinephrine exerts correlating effect on hemodynamics state providing the normalization of hemodynamic disorders of greater and lesser circulation. Obsidan and spesikor cause further aggravation of available hemodynamic disorders.

Study of respiration mechanics, relations of ventilation to blood flow and uniformity of alveolar ventilation in differential diagnosis of chronic cardiac and pulmonary insufficiency

Abstract

The respiratory system state in 52 patients with chronic cardiac diseases and cardiac insufficiency, in 64 patients with chronic pulmonary diseases and pulmonary insufficiency is studied. As many as 28 patients without cardiac and pulmonary diseases are examined as the control group. The group of patients with combination of cardiac and pulmonary diseases is distinguished. The reliability estimation of differences of the data obtained in the compared groups of patients is performed to determine the most reliable indices of external respiration function for differential diagnosis of chronic cardiac and pulmonary insufficiency. The indices of respiration median relations of ventilation to blood flow and uniformity of alveolar ventilation may be used in differential diagnosis of chronic cardiac and pulmonary insufficiency.

Abstract

The morphofunctional indices of blood cells in persons subjected to the action of chemical sensitizers in production of artificial leather are studied. The revealed quantitative and morphofunctional disorders depend on the duration of contact with chemical substances and clinical manifestations of toxicoallergy. These indices may be recommended as diagnosis criteria of toxicoallergic states to reveal thе risk group among workers maintaining contact with chemical substances.

Effect of operations in artificial circulation on luminoldependent chemoluminescence of blood

Abstract

The spontaneous hemoluminescence of blood and wound discharge in 26 patients operated in artificial circulation is studied. In uncomplicated course during artificial circulation and after operation independent of the effect of applied hemodilutants the increase of hemoluminescence of blood is found. Similar changes on days 2—3 after intervention are revealed in wound discharge. The indices approach to the initial values on days 4—5 after artificial circulation. Deviations from described dynamics coincide, as a rule, with grave postoperative state of patients. On the basis of the results of investigations the suggestions of the phagocytes activity changes in artificial circulation are made.

Rethoracotomie due то hemorrhage in early postoperative period after lung and mediastinum operations

Abstract

As many as 2458 lungs and mediastinum operations are performed in the clinic from 1967 to 1991 In 70 (2,8%) patients early postoperative period is complicated by intrapleural hemorrhage involving retocancerotmy. In all cases after the established diagnosis of prolonged intrapleural hemorrhage the urgest or postponed retocancerotomy is necessary. As many as 61 patients recovered, 9 patients died of 70 patients after retocancerotomy.

On the indications to surgical treatment of patients with hiatal hernias in combination with abdominal cavity organs diseases

Abstract

It is estalished that after surgical intervention the clinical manifestations of hiatal hernias are enhanced throughout the first year. The abdominal operation aggravates reflux-esophagitis. The limitations of indications to the operation based on the absence of visible in fibroesophagogastroscopy morphological changes are not always justified. Failure of supported antireflux mechanism even in uncomplicated forms of hiatal hernias in combination with abdominal cavity organs diseases involving surgical treatment is the indication to associative operative intervention, hernias elimination and correction of associative abdominal pathology.

The high rate of strain of ankle joint ligaments among strain of limbs joint ligaments is noted. The methods of their treatment depending on the manifestation of the clinical picture and complications are described. Appropriate, careful and by the time sufficient immobilization of joints allows early load on the limb, is accomponied by lesser pain, decreases the treatment terms and prevents the development of complications.

Content of prostaglandins in women with prolonged pregnancy and in their infants

Abstract

The study of the content of prostaglandins ir women with prolonged pregnancy and in their іnfants shows that their concentration is not distinguished from the results obtained in full-term pregnancy. It allows to consider prolonged pregnancy as physiologic with longer gestational term. Curantil, papaverine, imidazole, tocopherol are proposed for the correction of revealed disorders in prolonged pregnancy, trental and vitamin E are proposed for for prolonged infants.

Infant death rate is concerned with general amount of indicators of social-economic and écologie conditions of life. The basic causes of unfavourable tendencies of perinatal death rate are placental deficiency, unbearing of pregnancy, low mass of body in birth, revivification mistakes in connection with diagnosis difficulties and insufficient professional skill of physician and the absence of perinatal centres. The methods, aims and organization forms of the regional perinatal service are proposed.

Computer tomography in the examination complex of gynecologic patients

Abstract

Computer tomography is used to improve the diagnosis in examination of 26 gynecologic patients. The data obtained allow to differentiate ovarial tumors, myoma localization, to improve the nature of development defect as well as to reveal commissural process and intestinal loops conglomerate simulating ovarial tumors after previous laparotomy. The uterus neck stump, the postion and size of ovaries, localization of myoma nodes are clearly recognized on tomograms.

Effect of gas-polluted air on the heart function indices of boys aged 8-10 with different levels of motor activity

Abstract

The heart function indices of boys aged 8—10 living in gas-polluted air conditions and going in for sports systematically are studied. In intensified motor activity the shock and minute blood volumes increased significantly in both schools. In depressed motor activity the plausible changes of the heart function indices are found only in gas-polluted air condition.

Abstract

The processes of lipoperoxidation and the composition of erythrocytes membranes lipoids depending on liver affection aetiology (virus hepatitis A, virus hepatitis B) are studied. The revealed рronounced changes in the composition of lipoids and intensity of the processes of lipoperoxidation in virus hepatitis A as well as in virus hepatitis В have an effect on the realization of gasotransport function of the given cells. The differences between characteristics of patients with virus hepatitis A and virus hepatitis В may be used as additional criteria in differential diagnosis of virus hepatitis A and virus hepatitis B.

Abstract

It is noted that congenital arachnoid cysts arc not rare pathology of brain both in children and in adults and are characterized by hypertension—hydrocephalus and focal symptoms developing according to pseudotumarose type. The nucleomagnet and computer tomography are the most reliable methods oi their diagnosis. The patients with progradient and subcompensated dynamics of the disease course need surgical treatment.

The significance of human papillomavirus in the etiology and pathogenesis of cervical cancer

Abstract

The aim of our studies was to reveal the volume sequences of HPV type 16 and 18 oncogenic strains in women of Tatarstan treated for cervical cancer in the Oncology Research Center of Kazan. It turned out that all the 22 cancerous tumors studied contained mostly HPV type 16, in one case only HPV type 18 and in 2 cases a combination of types 16 and 18.

Abstract

Artificial ventilation (AV) is one of the most important methods of prosthetics of the main vital function of the body. Currently, the practice of anesthesiology, resuscitation and intensive care increasingly applies high-frequency ventilation. A number of papers describe its use through a catheter inserted directly into the trachea through a cricothyroid membrane (microtracheostomy - MT). Our clinic also has some experience of MT application (93 cases) in complex therapy of acute respiratory failure. It is used both for high-frequency ventilation, humidified O2 insufflation and for intratracheal administration of drugs.

Traumatic dislocation of extensor II and V fingers at the level of the metacarpophalangeal joint and its treatment

Abstract

The extensor tendons of fingers II and V on the back of the hand have intertendinous connections (connectors) that prevent their displacement during flexion-extension movements of the finger in the metacarpophalangeal joints. According to anatomical studies, the main phalanx is covered by spiral aponeurotic fibers from the back to the level of the metacarpophalangeal joint. However, the functional significance of this anatomical formation is not fully elucidated. As our clinical observations have shown, this formation carries a great functional load, especially for the II and V fingers of the hand.

Experience in treating children with perinatal lesions of the central nervous system using oriental massage

Abstract

We analyzed the effectiveness of treatment of children with various manifestations of perinatal damage to the central nervous system (brain and spinal cord) by the method of point, segmental and linear massage. From 1990 to 1991 we treated 29 children (boys - 17, girls - 12). From 4 weeks to 3 months there were 5 children, from 3 to 6 months - 7, from 6 to 9 months - 8, from 9 to 12 months - 3, from 1 to 3 years - 4, over 3 years - 2. Nineteen children were born from the first pregnancy, 7 from the second pregnancy, and 3 from the third pregnancy.

Over the past few years this problem has become even more urgent due to a more pronounced tendency to an increase in the number of pathological births and pregnancy complications, which has caused a significant reduction in the number of newborns corresponding to the 1st health group. The growing number of sick newborns, especially low-weight and premature newborns, requires large expenditures for their nursing.

Effectiveness of a new disinfectant composition for the prevention of nosocomial infections

Abstract

In the Yoshkar-Ola Children's Republican Hospital, disinfection is mainly carried out with chlorine-containing preparations (chlorine lime, chloramine, DP-2, etc.) and mostly by wet method - by sprinkling or dipping contaminated items in the disinfectant. However, these preparations have a significant disadvantage - high corrosive activity. In addition, they are toxic to humans and can accumulate in the environment. Because of their repeated use, bacteria become resistant.

Abstract

Infant death rate in rural area from 1985 to 1994 is analyzed. The rise of early neonatal death rate, the decrease of specific gravity of respiratory organs diseases and the increase of a share of perinatal period diseases; the rise of death rate of children under 1 year from congenital anomalies of development and infants under 1 month from respiratory disorders syndrome, birth injury, hypoxic and anoxic states are revealed. One of the ways in the resolution of the problems of premature children nursing is the creation of the republican perinatal centres conforming to the international standards as well as the improvement of current perinatal technologies and their introduction to practice.

As many as 10 thousand of liquidators of the accident consequences at the Chernobyl nuclear power station, other radiation accidents as well as more than one thousand of their children live in Tatarstan Republic at present. Medical and psychologic rehabilitation of liquidators of the accident at the Chernobyl nuclear power station and other radiation accidents in the special-purpose centre of medicosocial and psychologic rehabilitation using naturetherapy, psycho- and immunocorrection is optimum.

Abstract

The European Society of Cardiology, which now unites about 20,000 specialists from 40 countries, was founded in 1950 in Paris and met for the first time in London in 1952. The authors of the report first attended the XVIII Congress of the society, which was held this time in the center of England, in Birmingham, the National Exhibition Center, one of the world's largest facilities designed for conferences. The Congress was attended by 12,000 people and some 5,000 scientific presentations were made. Up to 10,000 delegates could be accommodated at the same time in 24 different conference rooms at this center and the adjoining Metropole Hotel. In addition, 5 gigantic halls were used for deployed exhibitions of modern cardiology equipment and medicines.
