Abstract

Since the production of pure cultures of sp. pallida, the question arose about the use of these cultures as an antigen in WaR. The possibility of using such an antigen seemed very tempting, since in this case, WaR would methodically fully satisfy the requirements of the Bordet-Gengou complement binding reaction, on the theoretical basis of which the WAR technique was developed by Wassermann, Neisser and Vgisk at the time.

Abstract

Each rational classification consolidates the knowledge and understanding of the studied phenomena achieved at the time of its compilation. Being the formulation of this understanding, the classification, in turn, becomes the starting point for further progress. That is why from time to time it is necessary to revise the nomenclature and classification of the studied phenomena in order to raise them to the level of new knowledge, in order to pour the accumulated new observations into them. In clinical medicine, the classifications of painful forms cannot, of course, have the accuracy and certainty that they have, for example, in the mathematical sciences. Insufficient knowledge of etiological factors, their multiplicity and interference, the abundance of transitional and combined forms, compensatory adaptations that equalize functional disorders-all this makes every step on the path of classification extremely difficult. However, if there is no data for making more or less complete classifications, it is often necessary to make working classifications that are obviously only temporary. These difficulties of classification especially increase when we are dealing with such vaguely differentiated painful phenomena as joint diseases. Here, an unimaginable "confusion" (Umber) prevails among practitioners. Practical doctors drown their inability to understand the intricate forms of disease in vague diagnoses of "rheumatism", "gout". Meanwhile, the great social significance that "rheumatic" joint diseases acquire-social diseases par excellence-requires more careful differentiation and consideration of joint diseases, and the latter are possible only when building a more or less rational classification.

Abstract

The question of the meaning of listening to the joints cannot be considered resolved. So far, we are in the stage of accumulating facts in this regard. In the literature, auscultation of the joints, as a method of research for the disease of the latter, has so far been given little attention. Not only the question of how valuable this research method is for the clinic is not sufficiently clarified, but also what data in the sense of sound phenomena we have when listening to normal and pathologically altered joints. Therefore, I consider it possible to report on the data that I received during auscultation of the knee joints in 373 people. The technique was as follows: a phonendoscope was placed on the knee joint area, after which the subject slowly performed flexion and extension movements while sitting on a couch (Prof. The doctor advises putting the patient's hands under the leg, this has not been done in my experiments). The movements were made repeatedly. Both joints were ausculted. Most patients have a hearing. it was performed several times (2-4) with different intervals: from a few days to 1½ months.

Transduodenal expulsion of tapeworms in children

Abstract

The wide application that the duodenal probe has found in clinical studies recently is well deserved. There is no need to dwell in detail on the great importance of this method in diagnostics, it is only necessary to note that in the therapeutic context, the clinic has been able to use a duodenal probe for the administration of a number of drugs, acting directly, in an unchanged form, on the 12-duodenum and neighboring organs (liver, bile ducts, pancreas). According to some authors, the duodenal probe was used to expel ribbon worms Gantt and West1), Schottmfiller 2), Schneider3), Kurakin4), Shafir 5) , etc. One of the first were the American authors Gantt and West, who used large doses of fern with great success. Schneider received a positive result out of 87 cases in 62 cases, he attached great importance to preliminary training and diet. The dose did not exceed 3.0 extra, fern. Kurakin-out of 88 cases, 83 received a positive result, the hoax was detected in 66 cases. The dose did not exceed 2.5 ferns. The day before, a laxative, an enema in the morning, then a duodenal probe was inserted and, when the olive was in the 12-duodenum, 25.0 inf was injected. Sennae, after ¼ hours of extr. fil. maris and then again 25.0 inf. Sennae. Kornev used large doses of extr. fil. maris up to 8.0; preliminary preparation lasted 2 days, and the removal of the worm was observed after 20-40 minutes. Shafir out of 40 cases in 100% showed the removal of the worm in an average of 2 21/2 hours. There was no preliminary preparation. The dose of fern in most cases did not exceed 4.0.

Abstract

Speaking about trachoma, it is impossible not to talk about the complications that it causes, because trachoma is terrible not so much by itself as by its complications. Among the complications of trachoma, the most common, apparently, are inversions and trichiasis. For 11,815 primary patients who passed through the outpatient clinic of the Kazan Trachomatous Institute in 1928, there were 1,926 people who were trachomatous, 317 of them with complications, i.e. 16.5%. According to Relman -) inversions complicate trachoma in the II-th stage in 30%, in the III-th in 50%, and according to the material of our Institute, there were 312 people (i.e. 42%) with a twist for 743 people diagnosed with trachoma III. This complication is very painful for the patient, because wrapped eyelashes rub against the eye and give a feeling of a foreign body. Without treatment, the inversion, causing pannous keratitis with subsequent opacity of the cornea, often leads to incurable blindness, which is why the inquisitive thought of optometrists gives so much time and energy to finding new ways to radically eliminate this complication. Back in the 16th century BC, the Egyptians proposed to treat the inversion of the eyelids with resets to the eye by pulling them out and applying a composition of myrtle, echidna blood and bat. The Indian Brahmin caste has been treating the inversion for many centuries before our era, cutting out strips of skin in the lower third of the upper eyelid and stitching the wound with hair sutures, and if this did not help, they burned out the bulbs of eyelashes , ). Without wanting to load our brief note with a stream of dates and names, we will not mention the numerous old and new, sometimes very ingenious ways that various authors have proposed at different times to quickly eliminate the inversion. Let us note only one thing, that a number of new operations, which we read about without exaggeration in almost every new volume of our special literature, serve as an indicator that existing operations do not fully satisfy oculists, and this same fact is an excellent illustration of the idea expressed by Prof. At a meeting of the Moscow Institute of Eye Doctors back in 1925, Odintsov said that with trichiasis "there is not a single completely satisfactory operation... that would radically eliminate changes, guarantee against relapses and give a perfect cosmetic effect"). The abundance of new operations, so generously offered to the overloaded practitioner, sometimes make them forget the old proven methods. So to say, to restore, to remind our workmates about one of these undeservedly forgotten ways of promptly eliminating trichiazama, we would like to use this brief note.

Do ulcers occasionally occur on the external female genitalia? which without bacterioscopic examination are dvagnossvruyug either as a soft chancre, or as a gangrenous ulcer. Not being in direct connection with intercourse, ONP more often affects older girls, virgins and only occasionally young women. I first noticed these ulcers in 1904. Lipschutz at the Finger clinic; during the bacteriological examination of smears from ulcers in and out of leukocytes, singly or in groups, he found Gram-positive short cells with crooked ends; later they were called bac. crassus; autoinoculation was negative. Lipschutz called these ulcers of the lodes noveneric; further observations of 4 cases in 1907-1909 allowed him in 1912 to establish a special nosological unit, to which he gave the name-ulcus vulvae acutum. A number of observations that appeared in the press, and most importantly 20 of his own, made it possible for the author to sharply distinguish this disease from other venereal and non—venereal ulcerative processes, such as those described in 1895 by Neumann aft of the female genital organs, or from the superficial ulcers of the anus and genitals described by Welan deg in women: these ulcers, despite similar features—soreness, development in virgins-differed in the development of vesicles on touching places etc.

To the casuistry of benign tumors of the gallbladder

Abstract

Benign neoplasms of the gallbladder are extremely rare (according to Finkelstein in 1 % of all surgically treated cases) and are detected, almost as a rule, accidentally during operations or autopsies. Most often, papillomas are observed in the gallbladder, which sometimes degenerate into cancer (P e ls-Leusden, HruSka, etc.); about 15 cases of them are described in the European literature. Fibroids, fibromyomas, fibroids, fibromyomas, papillary fibroepitheliomas, subserous lipomas, adenomas, cystadenomas and adenomyomas are much less common. The etiology of these neoplasms is still unknown: according to a number of authors (Savy, Maya, Finkelstein, etc.), the possibility of their development as a result of chronic rupture of the bladder wall by stones or infection is not excluded. Clinically benign tumors of the gallbladder quite often occur under the guise of chronic, recurrent cholecystitis or cholelithiasis.

Numerous factors play a role in the movement of blood through the veins, the residual strength of the heart, negative pressure in the chest cavity and the right heart are those moments that create a difference in the initial and final pressure in the veins and which provide siphon phenomena in them. At the same time, we should not forget about the role played by atmospheric pressure and due to which the phenomena of the siphon (Bogomolets) are possible. Among other factors contributing to the movement of blood through the veins, it is necessary to point out the venous valves and skeletal muscle contractions (Tigerstedt, Burton-Opitz). The significance of the latter can be seen at least from the fact that when drugs are injected into the veins, the patient is often forced to alternately clench his fingers into a fist and unclench them. However, the value of muscle contractions has recently been subjected to some authors to a certain doubt. So, according to Jones Wood, muscle contractions can contribute to blood flow only if they go evenly from segment to segment; but even if this were the case in an ideal case, then even then there could be stagnation and overflow of blood of the superficial veins, which, according to the author, are not exposed to the action of the muscular press.
