Resultsое distance lithotripsy depending on the pelvis system structure

Abstract

The results of the treatment of patients with urolithiasis by the distance piezoelectric lithotripsy method for 10 years are analyzed. It is established that the efficiency of this method depends not only on the sizes and physicochemical structure of concrements but mostly on their localization, the pelvis system structure type, anatomic and functional state of kidneys and upper urinary tract. It is enough to carry out. one procedure to destroy concrements of extrarenal pelvis, whereas the desintegration of concrements of intrarenal pelvis requires repeated procedures.

Efficiency of dimephosphonum, cytochrom cand mexidol in the treatment of cardiac rhythm disorders in children

Abstract

The efficiency of dimephosphonum, cytochrom c and mexidol in the treatment of ventricular, supraventricular arrhythmias in children is shown. Dimephosphonum (100 mg/kg i.v.), mexidol (10 mg/kg p.o.) and cytochrom c (0,5 mg/kg i.v.) were studied in comparison with cordaron, finoptin and also with “the standard neurometabolic therapy”. It is shown that dimephosphonum was the most effective drug in all kinds of disturbances of cardiac rhythm and conductivity in children. In supraventricular arrithmias and sinus node dysfunction it surpasses traditional drugs. At ventricular arrhythmias dimephosphonum was less effective than cordaron. However, cordaron induced cardiac (20%) and extracardiac (20%) negative effects. The dimephosphonum treatment was safe and was accompanied by favourable influence on the basic heart functions

Pump function of the heart in patients with scoliosis in rest and in stress test

Abstract

The indices of the pump function of the heart in teenagers with scoliosis of the 3d — 4 th degree in rest and during stress test are studied. Disorders of the pump function of the heart take place in rest: the heart rate signficantly increases. During physical exercise in patients with scoliosis the increase of minute blood volume is less pronounced than in healthy persons and is realized by the increase of the heart rate and not by the stroke blood volume. It is less economical form of adaptation of the pump function of the heart to physical exercise. The functional reserves for increasing the minute blood volume, volumetric circulation rate in the response to physical exercise are decreased

New method of epidural anesthesia in closed blunt chest injury

Abstract

The thoracic epidural anesthesia is the most suitable method for the treatment of the closed blunt chest injury but there is probability of the spinal cord injuiry. The original method combining the efficiency of classic thoracic epidural anesthesia with safety and simplicity of lumbar epidural anesthesia is suggested. This method is based on the postulates of the molecular hydrokinetic theory. Two catheters ends are disposed in lumbar epidural space in distance about two inches between them. Through one catheters end the anesthetic solution and through other’s end the 0,9% saline are injected synchronously in equal volumes. Humoral and cellular indices of immunity in newborns with early realization of the allergically changed reactivity

Abstract

The complex immunologic examination in 39 children aged 2 to 28 days with early realization of the allergically changed reactivity in boys — 61,5%, in girls — 38,5% is performed. The peculiarities of immunologic reactivity in children with early realization of the allergically changed reactivity (the increase of the immune regulatory index due to the deficiency of CD 8+ lymphocytes, the increased expression of activation markers, the increase of Ig E) show the prevalence of the atopic response type. Uniformity of changes of the immune status indices in early and late neonatal period testifies the intrauterine and/or early change of immunocompetent cell phenotype under the influence of perinatal development factors of a child

Peculiarities of hemodynamics in gestosis

Abstract

As many as 239 pregnant women suffering from gestosis were examined and they were allowed to give birth by the caesarean operation. Nine types of hemodynamics were selected. All the patients were divided into two groups, depending on the level of gestosis. In each group two subgroups were picked out according to the type of therapy (the 1st one by the old way, the 2d one by new methods). It is shown that in both groups the hypertensive-hypodynamic dissociation and isolated hypertension prevailed. It is stated that in both groups the calculation of the Cardiac Index and Systemic Vascular Resistance with the following definition of the hypodinamic type reflects the severity of the course of gestosis and the effeciency of the therapy directed to the correction of hemodinamics. The application of suggested systematization of hemodynamics disorders and the therapy methods promotes quicker correction of complications of hypertensive therapy in the form ofv postural reactions than in the case of the routine method.

Pathomorphological aspects of the placental deficiency in chlamidial And mycoplasmal infection

Abstract

The peculiarities of morphogenesis of placenta lesions and pathogenesis of placental deficiency in these types of infectious pathology are studied. It is stated that the subcompensated form of the relative chronic placental deficiency in chlamidiosis can be developed, morphological vascularization indices of ’ terminal villi being higher than normal and in mycoplasmosis such placental deficiency is connected with the partial decrease of vascularization indices. In the presence of mixed clamidia and mycoplasma infection in pregnants the dissociated type of villi lesions is observed and the placental vascularization indices (the content and state of capillaries, syncytiocapillar membranes and syncytial kidneys) ' are impaired to a greater extent resulting in chronic placental defficiency.

Clinical and immunohistochemical assessment of proliferative activity in uterine myoma

Abstract

A tumor of the uterus, consisting of smooth muscles, is called myoma and fibroids, and in practice, the terms are used synonymously. Myoma of the uterus is a common disease in women of childbearing age. The frequency of its detection without regard to age is 2.45%. With age, its prevalence increases and reaches 8.31% by the age of 50. The estimate of the final probability of contracting uterine fibroids in the population throughout life is 9.7%. According to sectional data, uterine fibroids, including small nodes, occur in 20% of women. In 50% of cases, these tumors are manifested by clinically pronounced disorders that lead a woman to a doctor.

A rare case of pseudotuberculosis with lethal outcome in the child aged two months and 20 days is described. Y. pseudotuberculosis infection of the child occurred likely in using milk, for the second time contaminated by the pseudotuberculosis pathogene. Within the last 60 years in the region studied the joint inhabitance zone of I. ricinus and I. persulcatus displaced in the North-East direction and the D. reticulatus natural habitat significantly expanded to the North- West. These regional changes depend on the temperature rise of climate, the decrease of precipitation and change of species and age of afforestation. Decay of epidemiologic stress of natural foci of the tick borne encephalitis, Lyme disease, and strengthening the epizootologic valency of foci of pyroplasmosis, nuttaliosis as well as epidemiologic f importance of natural foci of tularemia of meadowfield type would be expected in further expansion of the boundaries of I. ricinus, D. reticulatus natural habitat and contraction of the inhabitance zone of I. persulcatus on the territories cleared of the latter species.

Due to the increase in life expectancy in all countries, the stratum of elderly and senile people, who often have osteoporosis (OP), is increasing. The fragility of the bone base of the skeleton in OP leads to frequent fractures of the spine, femoral neck, bones  of the forearm and other areas of the musculoskeletal system (IOS). The prevention of OP and its treatment are becoming a socio-economic problem, in the solution of which doctors of all specialties should take part. In the course of studying the problem of OP, there has been a departure from the traditional ideas about it as an age-related inevitability, and in women as a result of menopause. It is appropriate to mention that S.A. Reinberg [2] emphasized the polyetiology of OP.

Abstract

At the present time, adaptive-reversible devices (ARA) have been created, designed to approximate the edges of the wound (WBR) in various areas of surgery [1, 8, I, 17, 19, 20, 21]. The first group includes tools for temporary sbkr, reducing its size and holding it in a certain position: tweezers, one- and two-pronged hooks, linen plate clamps or with a ratchet. The second group is made up of the most modern ARA for SBCR, allowing for intraoperative hardware correction during suturing, as well as during its long-term treatment. According to the design features of the working part and the converging mechanism, the ARA can be divided into the following main subgroups: 1st - needle (I), 2nd - plate (P), 3rd - filament (H), 4th - spoke (Cn ) and 5th - rod (St).
