Abstract

The clinical description of HIV-infection with Aids development in the child aged 5 is given. The peculiarity of the development of the acquired form of HIV- infection in children is shown: quickness and dynamics of the development of the basic process with general duration of the disease less than 5 years (from the moment of initiation of first symptoms up to fatal outcome), relatively short incubation and asymptomatic phase of the disease, the development sequence obliteration of discrete phases of the disease with the development of clinically manifestative Aids in second year of observation, rather prominent "bacterial component" of opportunist diseases, the presence of a great quantity of AIDS-assotiated infections.

Malignant course of tropical malaria

Autologic transplantation of the bone marrow: immediate and next results

Abstract

As many as 8 autologic transplantations of the bone marow are performed: 5 patients with hemoblastoses, 1 patient with lymphogranulomatosis and 2 patients with great tumors. The bone marrow is preserved in liquid nitrogen using the programmed freezer with home cryoconservator "Gekmolit". All patients are discharged. One patient with myelogenetic disease died in 70 days after transplantation and disease relapse. The state of the other 7 patients is satisfactory in 9—27 months.

Efficiency of sublingual specific immunotherapy in children with bronchial asthma and preasthma

Abstract

The efficiency of sublingual specific immunotherapy with everyday allergens of 112 children with respiratory allergoses — bronchial asthma and preasthma is studied. Its high efficiency is shown: persistant clinical remission of the disease is achieved in 88,4% of the children accompanied by the decrease of sensitization and hypersensitivity of the organ pertaining to shock to allergens and normalization of bronchial permeability.

Health of children being born premature

Abstract

The peculiarities of the health state of 90 children being bom premature of I—II degree are studied. The control group involved 90 children being mature in birth, of the same age, selected on the para-copy principle (sex and age). From the data of improved examination the high sickness rate, frequent psychical and behaviour disorders, retardation of physical development are revealed among children aged up to 14 with prematurity of I—II degree in birth.

Enzymodiagnostic importance of the lymphatic transport of lactate dehydrogenase, glutamate transaminase and alanin transaminase

Abstract

The hypothesis of the lymphatic system participation in enzymes transport from organs to blood is experimentally tested. The permeability changes of the external cell membrane of dogs muscular cells are achieved under the influence of the dosed physical load. Absolute and relative increase of the activity of lactate dehydrogenase, glutamate and alanin transaminase in lymph of the thoracal lymphatic duct is ahead of the same in blood. It is concluded that eriginally enzymes get to lymph and then to blood.

Clinico-rheoencephalographic correlations in deaf children

Abstract

The reasons of hearing disorder in children are studied. The neurologic and rheoencephalographic examination of 37 girls from the Medical and Pedagogical Center for children with hearing pathology aged 10—11 is performed. The signs of unheavy lesion of the cervical part of vertebral column and brain are revealed in 18 children. The signs of circulatory insufficiency in vertebrobasilar basin are revealed on rheoencephalograms in these children. It is proposed that the reason of deafness in many children is the natal trauma of the cervical part of vertebral column and vertebral arteries.

External respiration during treatment of patients with heart failure with peripheral vasodilators and strophantine

It is established that according to the efficiency the effect on external respiration function in shot-term treatment of patients with chronic cardiac insufficiency the drugs are ordered like: strophantin with capoten, strophantin, cordipin, capoten prazozin. In the treatment with isosorbide dinitrate the positive dynamics of external respiration function is not observed or its aggravation begins in most patients.

Internal picture of the vibratory disease and conditions of its formation

Abstract

The experimental and psychologic examination of 150 patients with the vibratory disease with mental disorders is performed. The predominant types of mental response to the given disease are revealed. Eigopatic (32%) and sensitive (22%) types of response or their combination (16%) are more often found in persons with boundary mental disorders, and ergopatic (42%) and harmonious (34%) types are more often found in persons without mental disorders. A number of social and psychologic and production factors such as conflicts, education in childhood, age, length of service affect the formation of one or other variant of the internal picture of the disease. The internal picture of the disease affects immediately social-labour adaptation of patients.

Abstract

The most often used method for the radiation diagnosis of labor traumas including traumas of the cervical part of vertebral column is roentgenography despite different current radiation diagnosis methods. The use of electroroentgenography and ultrasonic examination allows to reveal changes in osteal, disko- ligamentous and soft tissue structures in traumas of the cervical part of vertebral column. The original models of the trauma allow to realize the traumas initiation mechanism of the cervical part of vertebral column in different stages of labor, in obstetrical supplies and provide their prevention.

Morphology of cerebral cortex vessels in experimental epilepsy

Abstract

The reversible changes of cerebral cortex vessels prevail in cerebrovascular ischemia caused by unilateral ligation of the common carotid artery in rats. In bilateral ligation of the common carotid arteries, vertebral arteries, stenosis of ascending aorta the dystrophic vascular changes are manifested significantly. In postepileptic period in the presence of cerebrovascular disorders the dystrophic changes of cerebral cortex vessels prevail. In audiogenic epilepsies in the presence of renal hypertension and alimentary atherosclerosis the dystrophic changes of the vessels increase.

Peculiarities of the course and outcome of viral hepatitis A in children with acute dysentery Flexner's disease

Abstract

The aim of this work was to study the peculiarities of course and outcomes of viral hepatitis A (VHA) in children with acute dysentery Flexner's disease. The results of the study are based on clinical and laboratory examination data of Nizhny Novgorod Children's Hepatitis Center at Infectious Diseases Hospital No. 23 in 1989. 18 children at the age from 3 to 14 years old, who had and had recovered from VGA and acute dysentery Flexner's disease. Mild jaundice form of the disease was found in 2/3 of the children, and moderate - in 1/3 of the children. More than 2/3 of patients were boys of school age.

Autodermal plasty in surgery of postoperative ventral hernia

Abstract

In hernioplasty various grafts are used to close muscular-aponeurotic defects of the abdominal wall of various sizes, as well as to strengthen and replace weak and damaged anatomical gashes. The use of foreign materials for plasty of herniotomy gates often results in infiltrates, suppuration, and ligature fistulas. Extensive suppuration necessitates the removal of grafts (especially synthetic ones), which, as a rule, is complicated by hernia recurrence. In this regard, the advantages of using autografts, especially autodermal flaps due to their great strength, elasticity, resistance to infection are undeniable.

Complex treatment of a patient with multiple external intestinal fistulas after pancreatoduodenal resection

Abstract

The urgency of the problem of treatment of unformed external intestinal fistulas (UIE) is beyond doubt, since the overall mortality ranges from 2.8% to 34%, and postoperative mortality reaches 72%. The necessity of a complex therapy, the choice of elements of which is determined in each specific case, has been substantiated. An important component of this complex is measures to create and maintain hyperalimentation by combining various options of enteral and parenteral nutrition, which is the most acceptable in difficult economic conditions. Closure of large defects of the anterior abdominal wall seems to be no less important problem of NSCLC treatment and requires searching for new approaches in its solution.

Abstract

Closed abdominal and retroperitoneal injuries occur in 1.5-4% of peacetime injuries. Many issues of their diagnostics require further development. In this article we describe only clinical diagnosis of this pathology, without focusing on the use of instrumental methods (invasive and noninvasive). In our opinion, the clinical picture is of decisive importance in the diagnosis of any urgent abdominal pathology. Our observations are based on the analysis of 100 case histories of patients treated in the emergency surgery department of Kazan Emergency Hospital, as well as on the literature data.

Potentiated anesthesia in angiographic studies

Abstract

When performing X-ray endovascular studies (REI) under local anesthesia, patients often experience pain, which against the background of emotional stress leads to significant stress reactions. Concomitant arterial hypertension in patients with vascular pathology significantly increases the risk of angiographic examination (AI). In addition, patients are often in a forced position, with the chest compressed or in a supine position with a roller placed under the abdomen. Patients with low threshold of pain sensitivity and concomitant cardiovascular pathology cannot stay in such position for a long time. The choice of an adequate and low-toxic method of anesthesia is an important and urgent task in AI. This issue is practically not covered in the literature. In this summary, we describe our first experience with potentiated anesthesia in AI.

When examining them, there was a sign of weakness of adduction of the little finger due to paresis of its adductor muscle - segment C8.

Clinical-pharmacological approach to etiotropic therapy of acute pneumonia

Abstract

The purpose of the lecture is to encourage the general practitioner to act justifiably when choosing (means of treatment of pneumonia. It is necessary to abandon such a vicious scheme, when the patient is recommended to have injections of gentamicin or (!) lincomycin (antibiotics of different groups with significantly different antibacterial spectrum of action) only because they do not need to be done every 4 hours. Experience of advanced clinics of the world shows that gentamicin is used in the treatment of pneumonias not more often than 6% of cases, that tablet antibiotics are no less effective, that not all vitamin preparations can be prescribed during antibiotic therapy.
