Сердечно-сосудистый риск и перенесённая гестационная артериальная гипертензия у женщин репродуктивного возраста

Аннотация
В последние годы гестационная артериальная гипертензия, определяемая в соответствии с классификацией Европейского общества кардиологов (2011) как гипертензия, индуцированная беременностью, протекающая с протеинурией или без неё, развивающаяся после 20 нед беременности и исчезающая до 42 дней после родов, рассматривается в качестве прогностически неблагоприятного фактора в отношении развития сердечно-сосудистых заболеваний в последующем и смертности от них. В данном обзоре представлены результаты различных исследований по изучению взаимосвязи перенесённой гестационной артериальной гипертензии с факторами риска сердечно-сосудистых заболеваний, поражениями органов-мишеней, ассоциированными клиническими состояниями и сердечно-сосудистым риском у женщин, в том числе репродуктивного возраста. Определение индивидуального сердечно-сосудистого риска у женщин, особенно молодого возраста, затруднено ввиду низких чувствительности и специфичности тестов у данной категории пациенток. Несмотря на проведённые исследования и достаточный накопленный опыт, касающиеся проблемы по изучению взаимосвязи перенесённой артериальной гипертензии во время беременности с сердечно-сосудистым риском, до сих пор происхождение и вклад различных форм гипертензивных расстройств во время беременности, в том числе и гестационной артериальной гипертензии, в развитие будущих сердечно-сосудистых заболеваний остаются до конца неясными. Выявление перенесённой гестационной артериальной гипертензии позволит более точно оценивать индивидуальный сердечно-сосудистый риск и назначать адекватный комплекс лечебно-профилактических мероприятий женщинам, в том числе репродуктивного возраста, на более раннем этапе.
