There were 237 patients (54.0%) aged over 60 years and 89 (20.2%) aged over 70 years. A total of 259 patients underwent three positron emission tomography combined with computed tomography (PET/CT) scans (primary, interim, and final). Inclusion criteria were: age ≥18 years; no prior treatment for nonHodgkin lymphoma; and a histologically and immunohistochemically confirmed diagnosis. Exclusion criteria were: acute somatic or infectious diseases (including HIV infection) ; severe cognitive impairment; refusal to participate in the study; and residence outside the region. Statistical analysis was performed using IBM SPSS Statistics and Statistica 10 software. Normality of distribution was tested using the Kolmogorov–Smirnov test. Survival was assessed using the Kaplan–Meier method. Differences were considered statistically significant at <italic>р</italic> &lt; 0.05.</p> <p><bold>Results: </bold>Changes in the maximum standardized uptake value (SUVmax) on interim PET/CT were shown to be associated with the risk of progression and with overall survival rates, which decreased significantly with increasing disease stage. Assessment of the metabolic response at the interim stage using the Deauville scale demonstrated high prognostic significance: scores of 1–3 identified a group of patients with complete metabolic remission. However, even when a pronounced tumor response was achieved, some patients developed severe cumulative toxicity. Specifically, in a group of 120 patients who continued treatment after the fourth cycle, highgrade febrile neutropenia was recorded in 11 patients (9.2%), and in 10 of these cases (8.3%) it was accompanied by fatal complications due to multiple organ failure.</p> <p><bold>Conclusion: </bold>These findings underscore that in patients who achieve a complete metabolic response but have signs of severe cumulative toxicity, reducing the planned volume of chemotherapy should be considered to improve treatment safety.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование</bold>. В Российской Федерации наблюдается увеличение частоты злокачественных новообразований лимфоидной и гемопоэтической тканей, для которых, однако, не разработан единый алгоритм диагностики и лечения.</p> <p><bold>Цель исследования</bold>. Определить эффективные диагностические и лечебные подходы, а также критерии ранней адаптации терапии у пациентов с неходжкинскими лимфомами.</p> <p><bold>Методы</bold>. Ретроспективно изучены истории болезни 439 пациентов с двумя гистологическими типами неходжкинских лимфом, находившихся на стационарном лечении и наблюдении в ГАУЗ «РКОД» Минздрава Республики Татарстан в течение 10 лет: 386 пациентов с агрессивной лимфомой (диффузная В-крупноклеточная лимфома) и 53 — с индолентной (фолликулярная лимфома). Больных в возрасте старше 60 лет было 237 (54,0%), старше 70 лет — 89 (20,2%). У 259 пациентов выполнено три этапа позитронно-эмиссионной томографии, совмещённой с компьютерной томографией (первичная, промежуточная, заключительная). Критериями включения в исследование являлись: возраст старше 18 лет; отсутствие предшествующего лечения неходжкинских лимфом; наличие гистологически и иммуногистохимически подтверждённого диагноза. Критерии исключения: соматические и инфекционные заболевания (в том числе ВИЧ-инфекция) в стадии обострения; выраженный когнитивный дефицит; отказ от участия в исследовании; проживание в другом регионе. Статистическая обработка проведена с применением программного обеспечения IBM SPSS Statistics и Statistica 10. Нормальность распределения проверяли с помощью критерия Колмогорова–Смирнова. Выживаемость оценивали методом Каплана–Мейера. Различия считали статистически значимыми при <italic>р</italic> ≤0,05.</p> <p><bold>Результаты</bold>. Показано, что изменение максимального значения SUV (стандартизированного уровня накопления) на промежуточной позитронно-эмиссионной томографии, совмещённой с компьютерной томографией, связано с риском прогрессирования и показателями общей выживаемости, которая значимо снижалась с увеличением стадии заболевания. Оценка метаболического ответа на промежуточном этапе с использованием шкалы Deauville продемонстрировала высокую прогностическую значимость: значения 1–3 балла позволяли идентифицировать группу пациентов с полной метаболической ремиссией. Вместе с тем даже при достижении выраженного опухолевого ответа у части больных развивалась тяжёлая кумулятивная токсичность. Так, в группе из 120 пациентов, продолживших лечение после 4-го курса, фебрильная нейтропения высокой степени зарегистрирована у 11 (9,2%) пациентов, причём в 10 (8,3%) случаях она сопровождалась фатальными осложнениями на фоне полиорганной недостаточности.</p> <p><bold>Заключение</bold>. Полученные данные подчёркивают, что у пациентов, достигших полного метаболического ответа, но имеющих признаки тяжёлой кумулятивной токсичности, необходимо рассматривать возможность сокращения запланированного объёма химиотерапии для повышения безопасности лечения.</p></trans-abstract><trans-abstract xml:lang="zh"><p><bold>论证</bold>：在俄罗斯联邦，淋巴组织和造血组织恶性肿瘤的发病率呈上升趋势。然而，目前尚未针对该类疾病建立统一的诊疗方案，这限制了临床实践的规范化。</p> <p><bold>目的</bold>：本研究旨在确定非霍奇金淋巴瘤患者有效的诊断和治疗方法，并明确早期治疗调整（早期治疗干预）的标准。</p> <p><bold>方法</bold>：本研究回顾性分析了鞑靼斯坦共和国卫生部共和肿瘤专科医院10年间收治的439例非霍奇金淋巴瘤患者的病历资料。其中，侵袭性淋巴瘤（弥漫大B细胞淋巴瘤）患者386例，惰性淋巴瘤（滤泡性淋巴瘤）患者53例。患者中，60岁以上者237例（54.0%），70岁以上者89例（20.2%）。259例患者进行了三个阶段的正电子发射断层扫描/计算机断层扫描检查（首次、中期、末次）。纳入标准：年龄大于18岁；既往未接受过非霍奇金淋巴瘤治疗；具有经组织学及免疫组织化学证实的诊断。排除标准：躯体及感染性疾病（含HIV感染）处于急性期者；存在明显认知功能障碍者；拒绝参与研究者；居住地为外地者。统计学分析采用 IBM SPSS Statistics 和 Statistica 10 软件。正态分布检验使用 Kolmogorov-Smirnov 检验。采用 Kaplan-Meier 方法评估生存率。<italic>p</italic>≤0.05 时差异具有统计学意义。</p> <p><bold>结果</bold>：研究表明，中期正电子发射断层扫描/计算机断层扫描中最大标准摄取值的变化与疾病进展风险及总生存率相关，且生存率随疾病分期的增加而显著降低。利用Deauville标准评估中期代谢反应具有极高的预测价值：评分为1-3分的患者可被认定为代谢完全缓解组。然而，即使在部分患者取得显著肿瘤缓解的情况下，仍出现了严重的累积毒性。在第4周期治疗后继续进行治疗的120例患者中，11例（9.2%）出现高度发热性中性粒细胞减少症，其中10例（8.3%）在多器官功能衰竭背景下发生致死性并发症。</p> <p><bold>结论</bold>：研究结果表明，对于已达到完全代谢缓解但呈现严重累积毒性迹象的患者，应考虑减少既定化疗剂量或周期，以提高治疗安全性。</p></trans-abstract><kwd-group xml:lang="en"><kwd>nonHodgkin lymphoma</kwd><kwd>positron emission tomography combined with computed tomography</kwd><kwd>fiveyear survival</kwd><kwd>Deauville scale</kwd><kwd>immunochemotherapy</kwd><kwd>toxicity</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>неходжкинская лимфома</kwd><kwd>позитронно-эмиссионаая томография, совмещённая с компьютерной томографией</kwd><kwd>пятилетняя выживаемость</kwd><kwd>шкала Deauville</kwd><kwd>иммунохимиотерапия</kwd><kwd>токсичность</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>非霍奇金淋巴瘤</kwd><kwd>正电子发射计算机断层成像</kwd><kwd>五年生存率</kwd><kwd>Deauville标准</kwd><kwd>免疫化疗</kwd><kwd>毒性</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Casasnovas RO, Ysebaert L, Thieblemont C, et al.
