However, the indications for this intervention remain debatable.</p> <p><bold>AIM</bold>: To review early experiences with primary anastomosis for diverticular abscesses.</p> <p><bold>METHODS</bold>: The treatment outcomes of 58 patients with diverticular abscesses were studied. The evaluated abscesses were located in pericolic (32 [55.2%] patients), pelvic (19 [32.7%]), and distant sites (7 [12.1%]). Treatment included drug therapy, minimally invasive interventions, and combined surgical interventions. Treatment outcomes in 26 patients who underwent bowel resection with primary anastomosis and 11 patients who underwent Hartmann’s procedure were compared. Significant differences were assessed using Fisher’s exact test and Student’s <italic>t</italic>-test for independent samples using the GraphPad QuickCalcs online calculator based on standard statistical analysis algorithms.</p> <p><bold>RESULTS</bold>: Primary anastomosis was performed in 12 patients (37.5%) with pericolic abscesses, 11 patients (57.9%) with pelvic abscesses, and 3 patients (42.9%) with distant abscesses. The surgical option was selected based on the abscess’ location and extent and the patient’s condition, age, and comorbidities. In patients with primary diverticular abscesses who underwent urgent surgeries, anastomosis was conducted in 10 of 21 (47.6%) patients, and in all 16 patients with recurrent abscesses. Young and middle-aged individuals (55.5 ± 9.42 years) predominated in the primary anastomosis group, while elderly and senile individuals (71.6 ± 8.94 years) predominated in the stoma group. Bowel resection, although more technically complex than Hartmann’s procedure, showed no significant increase in complications (<italic>p</italic> = 0.119) and resulted in less severe outcomes.</p> <p><bold>CONCLUSION</bold>: Primary anastomosis for diverticular abscesses is a surgical option for young and middle-aged individuals without comorbidities and generalized inflammation.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование</bold>. В последнее 10-летие появились публикации о возможности формирования первичного колоректального анастомоза при гнойно-воспалительных осложнениях дивертикулярной болезни, однако показания к выполнению таких вмешательств остаются предметом дискуссии.</p> <p><bold>Цель исследования</bold>. Оценить начальный опыт применения первичного анастомоза при абсцессах дивертикулярного генеза.</p> <p><bold>Методы</bold>. Проведено когортное исследование результатов лечения 58 пациентов с абсцессами дивертикулярного генеза. Локализация абсцессов: периколические — у 32 (55,2%) пациентов, тазовые — у 19 (32,7%) и отдалённые — у 7 (12,1%). Лечение включало консервативную терапию, малоинвазивные манипуляции и комбинированные оперативные вмешательства. Проведено сравнение результатов лечения 26 пациентов, которым была выполнена резекция кишки с формированием первичного анастомоза, и 11 пациентов, оперированных по типу Гартмана. Статистическую значимость различий оценивали с использованием точного критерия Фишера и критерия Стьюдента для независимых выборок с помощью онлайн-калькулятора GraphPad QuickCalcs, основанного на стандартных алгоритмах статистического анализа.</p> <p><bold>Результаты</bold>. Первичный анастомоз сформирован у 12 (37,5%) пациентов с периколическими абсцессами, 11 (57,9%) — тазовыми и 3 (42,9%) — отдалёнными. Выбор варианта операции зависел от локализации и распространённости абсцесса, тяжести состояния пациента, возраста и наличия коморбидной патологии. У пациентов с первичными дивертикулярными абсцессами, оперированных в неотложном порядке, анастомоз сформирован у 10 (47,6%) из 21, тогда как у 16 пациентов с рецидивными абсцессами анастомозирование выполнено у всех. Среди пациентов с первичным анастомозом преобладали лица молодого и среднего возраста (55,5 ± 9,42 года), а среди стомированных пациентов — лица пожилого и старческого возраста (71,6 ± 8,94 года). Выполнение резекции кишки, будучи технически более сложной операцией по сравнению с вмешательством типа Гартмана, не сопровождалось увеличением числа осложнений (<italic>p</italic> = 0,119), причём отмеченные осложнения носили менее тяжёлый характер.</p> <p><bold>Заключение</bold>.
