The patients were divided into two groups. The control group included 52 patients (35 [67.3%] males and 17 [32.7%] females, mean age: 50.9 [SD ± 15.0] years) who received standard of care. The treatment group included 94 patients (69 [73.4%] males and 25 [26.6%] females, mean age: 46.6 [SD ± 9.5] years) who additionally received enteral therapy using an enteral saline solution. The solution’s composition was identical to that of chyme from the first part of the small intestine in apparently healthy individuals. Its acidic environment (pH 5.0–5.8) has a bacteriostatic effect and inhibits the growth of opportunistic and pathogenic microorganisms. Enteral therapy was provided via a nasointestinal tube, which was placed along the endoscopic instrument channel and attached to the ligament of Treitz. The solution (1500 mL) was administered at a rate of 6–10 mL/min with intra-abdominal pressure monitoring, which did not exceed 16 mmHg. The Mann–Whitney <italic>U</italic> test was used for intergroup comparisons. Differences were considered significant at <italic>p</italic> &lt; 0.05.</p> <p><bold>RESULTS</bold>: The treatment group showed a decrease in intestinal injury marker (I-FABP) levels the day after starting enteral therapy. On days 2 and 7, there was a significant decrease in intestinal fatty acid binding protein (I-FABP) levels to 1.52 ng/mL (<italic>p</italic> = 0.049) and 1.42 ng/mL (<italic>p</italic> = 0.01), respectively. These values were comparable to the median I-FABP level in the control group (1.40 ng/mL), which was used as a reference. On day 2, the threshold value was comparable to the reference (<italic>p</italic> = 0.049), with a significant decrease on day 7 (<italic>p</italic> = 0.01).</p> <p><bold>CONCLUSION</bold>: The findings confirm the efficacy of the enteral saline solution in patients with severe acute pancreatitis.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование</bold>.<bold> </bold>Микроциркуляторные нарушения в кишечнике у пациентов с тяжёлым острым панкреатитом ведут к нейроэндокринной дисрегуляции, дисфункции кишечных эпителиоцитов, нарушению моторной и эвакуаторной функций кишечника.</p> <p><bold>Цель исследования</bold>.<bold> </bold>Оценить эффективность энтерально-корригирующей терапии синдрома кишечной недостаточности у пациентов с тяжёлым острым панкреатитом с использованием солевого энтерального раствора.</p> <p><bold>Методы</bold>. В мультицентровое нерандомизированное клиническое исследование включено 146 пациентов с тяжёлым острым панкреатитом [112 (76,7%) мужчин и 34 (23,3%) женщины], средний возраст — 50,8 (SD ±13,7) года. Пациенты разделены на две группы. Группу сравнения: 52 пациента [35 (67,3%) мужчин и 17 (32,7%) женщин, средний возраст — 50,9 (SD ±15,0) года], получавшие стандартную терапию. Основная группа: 94 пациента [69 (73,4%) мужчин и 25 (26,6%) женщин, средний возраст — 46,6 (SD ±9,5) года], лечение которых дополнено энтеральной терапией с использованием солевого энтерального раствора. Раствор по составу идентичен химусу начальных отделов тонкой кишки условно здорового человека. Его кислая среда (рН 5,0–5,8) обладает бактериостатическим эффектом и подавляет рост условно-патогенной и патогенной микрофлоры. Энтеральную терапию проводили через назоинтестинальный зонд, установленный по инструментальному каналу эндоскопа за связку Трейтца. Раствор вводили со скоростью 6–10 мл/мин в объёме 1500 мл под контролем внутрибрюшного давления, которое не превышало 16 мм рт. ст. Сравнение групп проводили с применением U-критерия Манна–Уитни. Статистическую значимость различий оценивали при уровне <italic>p</italic> &lt;0,05.</p> <p><bold>Результаты</bold>.<bold> </bold>У пациентов основной группы уже через сутки от начала энтеральной терапии отмечалась тенденция к снижению маркёра кишечного повреждения (I-FABP). На 2-е и 7-е сутки наблюдалось статистически значимое снижение интестинального белка, связывающего жирные кислоты (I-FABP), до 1,52 нг/мл (<italic>p</italic>=0,049) и 1,42 нг/мл (<italic>p</italic>=0,01) соответственно. Эти значения приближались к медианному уровню I-FABP участников контрольной группы (1,40 нг/мл), принятому в качестве референсного.
