It is an uncommon neoplasm with unpredictable biological behavior and varying treatments. Despite its histologic features indicative of benignity, including a low proliferative index, its clinical course is frequently malignant, with a high local recurrence rate and metastatic potential. The distinctive characteristics of malignant melanotic nerve sheath tumor include a relatively young patient age, signs of Carney complex, and psammoma bodies within the tumor. This study presents a case of a patient with malignant melanotic nerve sheath tumor located in the neck with a recurrent clinical course who underwent two surgical procedures. Microscopic and immunohistochemical evaluation of postoperative specimens showed positivity for melanoma markers, absence of psammoma bodies, and low mitotic activity. The clinical course of the disease was malignant, with recurrence developing in the postoperative scar. Diagnosing malignant melanotic nerve sheath tumor is radiologically and pathologically challenging. Multidisciplinary collaboration among the surgeon, radiologist, and pathologist; understanding the lesion’s anatomical context; and close attention to histopathologic findings are critical for establishing an accurate diagnosis.</p></abstract><trans-abstract xml:lang="ru"><p>Злокачественная меланотическая опухоль оболочки нерва, ранее известная как меланотическая шваннома, — редкий вариант опухоли оболочки периферического нерва, состоящей из шванновских клеток, которая продуцирует меланин и клинически ошибочно диагностируется как меланома. Злокачественная меланотическая опухоль оболочки нерва бывает редкой опухолью с неопределённым биологическим поведением и различными рекомендациями по лечению. Несмотря на гистологические признаки, кажущиеся доброкачественными, с низким индексом пролиферации, клиническое течение этой опухоли нередко становится злокачественным, с высокой частотой местных рецидивов и наличием признаков метастазирования опухоли. Отличительные особенности злокачественной меланотической опухоли оболочки нерва — относительно молодой возраст пациентов, наличие признаков комплекса Карни и псаммоматозных телец в опухоли. В статье представлен случай злокачественной меланотической опухоли оболочки нерва, локализованной на шее, с рецидивирующим течением. В описанном клиническом наблюдении пациенту дважды было выполнено хирургическое лечение. Микроскопический и иммуногистохимический анализ послеоперационных материалов дал положительный результат на маркёры меланомы и показал отсутствие тел псаммомы и низкую митотическую активность. Клиническое течение заболевания оказалось злокачественным, с появлением рецидива в послеоперационном рубце. Диагностика злокачественной меланотической опухоли оболочки нерва — сложная задача, как с рентгенологической, так и с патологической точки зрения.
