The analysis of the current state of views on a pathogenesis, classification and features of a clinical picture of syncope of cardiogenic etiology was performed. Cardiogenic syncope has a 20-30% mortality. The highest risk is characteristic for the age more than 45 years, heart failure, history of ventricular tachycardia, nonspecific changes on ECG. With three and more risk factors the probability of a sudden cardiac death within the next year can reach 80%. The cause of cardiogenic syncope is reduction of cerebral blood flow. Cardiogenic syncope can be caused by a fall in blood pressure, cardiac output and total peripheral vascular resistance. Cardiogenic syncope may be the result of bradycardia and asystole, supraventricular and ventricular tachycardia, pulmonary embolism, dissecting aortic aneurysm, structural defects and cardiac anomalies, presence of myxoma or mobile atrial clot, cardiac tamponade, constrictive pericarditis, acute myocardial ischemia, abnormalities of the coronary arteries, dysfunction of prosthetic valve and cardiac pacemakers. In the diagnosis of cardiogenic syncope anamnesis and data received on physical examination are important. Instrumental examination should include electrocardiography, detection of comorbid conditions, exercise stress tests such as cycle ergometry or treadmill test, Holter ECG monitor, echocardiography, and coronary angiography for suspected acute myocardial ischemia.</p></abstract><trans-abstract xml:lang="ru"><p>Представлен обзор современных публикаций, посвящённых этиологии, диагностике и лечению синкопальных состояний кардиогенной этиологии. Проведён анализ современного состояния взглядов на патогенез, классификацию и особенности клинической картины синкопальных состояний кардиогенной этиологии. Летальность при кардиогенных синкопальных состояниях составляет 20-30%. Наибольший риск представляют возраст старше 45 лет, сердечная недостаточность, желудочковая тахикардия в анамнезе, неспецифические изменения на электрокардиограмме. При наличии трёх и более факторов риска вероятность внезапной сердечной смерти в течение следующего года может достигать 80%. Причина кардиогенных синкопальных состояний - снижение мозгового кровообращения. Кардиогенные синкопальные состояния могут быть вызваны падением артериального давления, минутного объёма и общего периферического сосудистого сопротивления. Эти состояния могут быть следствием брадикардии и асистолии, суправентрикулярной и желудочковой тахикардии, тромбоэмболии лёгочной артерии, расслаивающей аневризмы аорты, структурных нарушений и пороков сердца, наличия миксомы или подвижного тромба в предсердии, тампонады сердца, констриктивного перикардита, острый ишемии миокарда, аномалии коронарных артерий, дисфункции клапанных протезов и кардиостимуляторов. В диагностике кардиогенных синкопальных состояний важны данные анамнеза и физикального обследования. Инструментальное обследование планируют в следующем объёме: электрокардиография, диагностика сопутствующих патологических состояний, тест с физической нагрузкой в форме велоэргометрии или тредмил-теста, холтеровское мониторирование электрокардиограммы, эхокардиография и коронарная ангиография при подозрении на острую ишемию миокарда.</p></trans-abstract><kwd-group xml:lang="en"><kwd>syncope</kwd><kwd>diagnosis</kwd><kwd>treatment</kwd><kwd>arrhythmia</kwd><kwd>electrocardiography</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>синкопальные состояния</kwd><kwd>диагностика</kwd><kwd>лечение</kwd><kwd>аритмия</kwd><kwd>электрокардиография</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Бова А.А.
