of immunosuppressive cyclophosphamide therapy and its regimens on the rate of progression of chronic kidney disease in mesangioproliferative glomerulonephritis.</p> <p><bold>Methods.</bold> 72 patients with mesangioproliferative glomerulonephritis and indications for immunosuppressive therapy with disease activation were included in the comparative analysis: 56 patients received cyclophosphan in conventional doses (26 patients with daily or every other day, 30 patients with in pulse mode 1 time per month), and 16 patients did not receive cyclophosphan. Duration of the disease before observation ranged from 0 to 33.58 years, a median follow-up was 6.00 (interquartile range 1.63–13.17) years, and after observation from 0 to 5 years with the median follow-up was 2.00 (1.00–3.50) years. The examination included nephrobiopsia with a morphological diagnosis, activity index/sclerosis, and glomerulonephritis progression rate for decreased glomerular filtration rate (ml/min/1.73 m² per year).</p> <p><bold>Results.</bold> The progression rate of chronic kidney disease was higher in the group of patients not receiving immunosuppressive therapy, 5.57 (3.27–7.95) ml/min/1.73 m2 per year compared with of the treated patients group, 3.05 (2.04–6.78) ml/min/1.73 m2 per year (p=0.040). There were no differences in the rate of decrease in glomerular filtration rate between groups depending on the treatment regimen: 4.86 (2.12–6.77) ml/min/1.73 m2 per year with regular and 3.67 (2.04– 6.91) ml/min/1.73 m2 per year with a pulse mode (p=0.720). The rate of glomerulonephritis also did not differ significantly: 1.0 (1.0–2.0) and 2.0 (1.0–2.0) relapses over 5 years, respectively (p=0.691) in both treatment regimens.</p> <p><bold>Conclusion.</bold> The treatment of patients with mesangioproliferative glomerulonephritis with cyclophosphane, in combination with prednisone or without it, regardless of the treatment regimen induces a slowdown in the progression of chronic kidney disease, improving the long-term prognosis and without affecting the frequency of relapses of the disease.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель.</bold> Оценка влияния иммуносупрессивной терапии циклофосфаном и её режимов на темпы прогрессирования хронической болезни почек при мезангиопролиферативном гломерулонефрите.</p> <p><bold>Методы.</bold> 72 пациента с мезангиопролиферативным гломерулонефритом и показаниями к иммуносупрессивной терапии при активизации заболевания, из которых 56 получали циклофосфан в общепринятых дозах (26 человек — ежедневно или через день, 30 пациентов — в пульс-режиме 1 раз в месяц), 16 пациентов не получали циклофосфан. Длительность заболевания до взятия под наблюдение составила от 0 до 33,58 года, медиана 6,00 (1,63–13,17) лет, после него — от 0 до 5 лет c медианой 2,00 (1,00–3,50) года. Обследование включало нефробиопсию с установлением морфологического диагноза, индекса активности/склероза и темпов прогрессирования гломерулонефрита по снижению скорости клубочковой фильтрации (мл/мин/1,73 м2 в год).</p> <p><bold>Результаты.</bold> Темпы прогрессирования хронической болезни почек были более высокими в группе пациентов, не получавших иммуносупрессивную терапию, — 5,57 (3,27–7,95) мл/мин/1,73 м2 в год по сравнению с группой пролеченных пациентов — 3,05 (2,04–6,78) мл/мин/1,73 м2 в год (р=0,040). Не выявлено различий в темпах снижения скорости клубочковой фильтрации между группами в зависимости от режима терапии: 4,86 (2,12–6,77) мл/мин/1,73 м2 в год при регулярном и 3,67 (2,04–6,91) мл/мин/1,73 м2 в год при пульс-режиме (p=0,720). Это же касается частоты рецидивов гломерулонефрита: соответственно 1,0 (1,0–2,0) и 2,0 (1,0–2,0) рецидива за 5 лет (p=0,691) при обоих режимах терапии.</p> <p><bold>Вывод.</bold> Лечение пациентов с мезангиопролиферативным гломерулонефритом циклофосфаном, в комбинации с преднизолоном или без таковой, независимо от режима терапии индуцирует замедление темпов прогрессирования хронической болезни почек, улучшая отдалённый прогноз и не оказывая при этом влияния на частоту рецидивов заболевания.</p></trans-abstract><kwd-group xml:lang="en"><kwd>mesangioproliferative glomerulonephritis</kwd><kwd>cyclophosphamide</kwd><kwd>modes of therapy</kwd><kwd>rate of progression of chronic kidney disease</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>мезангиопролиферативный гломерулонефрит</kwd><kwd>циклофосфан</kwd><kwd>режимы терапии</kwd><kwd>темпы прогрессирования хронической болезни почек</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Bikbov B.T., Tomilina N.A.
