25 (41.7%) pregnant women, included in the control group, were delivered off by urgent cesarean section. Considering the goal of the study, bacteriology of vaginal discharge, amniotic fluid and placental tissues, DNA detection of vaginal discharge bacterial flora before and after treatment (using «Femoflor-16» test system) according to classification by M.N. Boldyreva (2010), uterine cavity aspirate cytology according to classification by M.A. Kupert (2003), sonography of the uterus and uterine adnexa on the 4-5 day after the surgery according to classification of A.P. Milovanov were added to the training program for pregnant women. In the study group, pregnant patients with severe anaerobic vaginal dysbiosis at the 36-38 weeks of gestation were administered causal treatment of severe anaerobic vaginal dysbiosis, pregnant patients of the control group were not treated if severe anaerobic vaginal dysbiosis was diagnosed.</p><p><bold>Results.</bold> Performed correction of vaginal microbiota in the study group has significantly improved the course of postoperative period, reduced the incidence of uterine subinvolution, signs of chorioamnionitis as a manifestation of ascending infection compared to the control group. Postpartum endometritis has developed in 1 (2.8%) patient of the study group and in 5 (20.0%) cases in the control group. Offered method for preparing the patients with vaginal dysbiosis to cesarean section reduced the risk for postpartum endometritis by 6.8 times.</p><p><bold>Conclusion.</bold> Strategy of preparation for planned Caesarean section should include vaginal discharge bacteriology and treatment of severe anaerobic vaginal dysbiosis, which reduces the risk for postpartum endometritis by 6.8 times.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель.</bold> Профилактика гнойно-септических осложнений у женщин после планового оперативного родоразрешения путём кесарева сечения.</p><p><bold>Методы.</bold> Нами проведено комплексное обследование 60 беременных, из которых 35 (58,3%) составили исследуемую группу и были родоразрешены оперативным путём в плановом порядке. Группу контроля составили 25 (41,7%) беременных, родоразрешённых путём операции кесарево сечение в ургентном порядке. С учётом поставленной цели программа подготовки беременных к плановому кесареву сечению была дополнена следующими исследованиями: бактериологическое исследование влагалищного содержимого, околоплодных вод и плацентарной ткани; генетическая диагностика микробного спектра влагалищного содержимого до и после лечения (тест-системой «Фемофлор-16») по классификации М.Н. Болдыревой (2010); цитолологическое исследование аспирата из полости матки по классификации М.А. Куперт (2003); ультразвуковое исследование матки и придатков на 4-5-е сутки послеоперационного периода по классификации А.П. Милованова (2006). В исследуемой группе беременным с выраженным анаэробным влагалищным дисбиозом в сроке 36-38 нед назначали этиотропную терапию анаэробного влагалищного дисбиоза, беременным контрольной группы при выявлении анаэробного влагалищного дисбиоза лечение не проводили.</p><p><bold>Результаты.</bold> Проведённая коррекция влагалищного биоценоза в исследуемой группе позволила значительно улучшить течение послеоперационного периода, снизить частоту субинволюции матки и развития признаков хориоамнионита, как проявления восходящей инфекции, по сравнению с группой контроля. Развитие послеродового эндометрита в исследуемой группе зарегистрировано у 1 (2,8%) пациентки исследуемой группы и в 5 (20,0%) случаях в группе контроля. Предложенная методика подготовки пациенток с дисбиозом влагалища к проведению кесарева сечения позволила в 6,8 раза снизить риск возникновения послеродового эндометрита.</p><p><bold>Вывод.</bold> Стратегия подготовки к плановому кесареву сечению должна включать исследование патологии микробного спектра влагалищного содержимого и проведение его коррекции по результатам обследования, что позволяет снизить в 6,8 раза риск развития послеродового эндометрита.</p></trans-abstract><kwd-group xml:lang="en"><kwd>vaginal dysbiosis</kwd><kwd>diagnosis</kwd><kwd>prevention</kwd><kwd>pharmacological treament</kwd><kwd>planned caesarean section</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>влагалищный дисбиоз</kwd><kwd>диагностика</kwd><kwd>профилактика</kwd><kwd>медикаментозная коррекция</kwd><kwd>плановое кесарево сечение</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Гуртовой Б.Л., Кулаков В.И., Воропаева С.Д.
